**[Chapter 17 ]** **HIV** - Patho - Caused by retrovirus - Cannot infect other cells unless it highjacks a cell of the person whose body they are in - Can progress to aids - Process - Virus takes over CD4 cells - Inserts a single strand of RNA makeup into the hosts DNA (retrovirus) - HIV enzyme, reverse transcript, converts the virus RNA to DNA - HIV virus enzyme, \_\_\_\_\_\_\_\_\_\_, is then used to insert DNA into host cell's nucleus - HIV particles are made into long strand and clipped by HIV protease - New particles buds off looking for other CD4 cells to infect - Difference between HIV and AIDs - Viral load - Specific lab -- the CD4 count under 200 = AIDS - HIV has progressed to AIDS when the CD4 count is \3 seconds - Are they dehydrated? Or at risk? - Are they in fluid overload? - Are their extremities cold? - Priority circulation related nursing diagnosis - Appropriate cardiac output - Fluid balance - F&E - Disability (think neuro) and safety related questions to ask yourself - Is the client a&ox4 - Is the client able to make good decisions? - Does the client understand their limitations (do they know when to call for help) - Priority disability/safety related nursing diagnosis - Lack of oxygen to the brain **Psychosocial client needs** - Physical problems come before psychosocial/teaching needs! - Actual problems come before RISK FOR problems - Psychosocial clients need examples - Pain - Body image issues - Anxiety - Depression - Anger - Lack of support system - Lack of sleep - Lack of resources -- if someone does not have a job or means of income
