Study Notes
Ventolin Overview
- Generic names for Ventolin include Salbutamol and Albuterol Sulfate.
- Classified as a bronchodilator and a beta2-selective adrenergic agonist (sympathomimetic).
Forms of Supply
- Available as 2.5 mL nebule (1 mg/mL) and metered-dose inhaler (MDI) with 100 mcg/spray.
- Combination therapy includes Combivent, which contains Ventolin (2.5 mg) and Atrovent (500 mcg).
Mechanism of Action
- Selectively stimulates beta-adrenergic receptors in the lungs, uterus, and vascular smooth muscle.
- Promotes bronchodilation by relaxing vascular smooth muscles, reducing airway resistance and bronchospasm.
- Higher doses may lead to shifting of serum potassium (K+) into cells.
Indications for Use
- Effective for bronchospasm in asthma, chronic bronchitis, and other chronic broncho-pulmonary disorders, as well as in anaphylaxis.
- Treats hyperkalemia by promoting cellular uptake of potassium.
Dosage Information
- Adult dose for bronchospasm: 5.0 mg via neb, repeat every 10 minutes as needed; or 10 puffs with a spacer every 5 minutes using MDI.
- Adult dose for hyperkalemia: 10-20 mg nebulized over 10 minutes; if starting at 1 mg/mL, begin with 5 mg nebulized continuously until the total dose is reached.
-
Pediatric dose: 0.15 mg/kg diluted to 2.5 mL saline via nebulizer; specific dosing based on weight:
- <10 kg: 1.25 mg with NS to 2.5 mL
- 10-20 kg: 2.5 mg
-
20 kg: 2.5 - 5.0 mg
- MDI usage: minimum of 2 puffs, maximum of 10.
Contraindications
- Avoid in cases of hypersensitivity to Ventolin or its components.
- Not recommended for patients with tachydysrhythmias.
Precautions
- Should not be used for cardiac asthma management.
- Caution advised in individuals with cardiovascular disease, including cardiac dysrhythmias and hypertension.
- Use with caution in patients with diabetes mellitus due to the risk of drug-induced hyperglycemia.
- Hypokalemia presents a risk as Ventolin may further lower serum potassium levels, potentially leading to adverse cardiovascular events.
