Standard Operating Procedure for Vehicle Accidents
- Aim: Provide a systematic procedure for evaluation, stabilization, and operation during vehicle accidents necessitating patient extrication.
- Responsibility: Every unit Officer-in-Charge (OIC) must ensure crew wears complete turnout gear, including eye and hand protection.
- Bloodborne pathogens must be addressed to protect all personnel.
Size-Up Phase
- First arriving unit OIC to perform a size-up, delegating inner and outer surveys.
- Minimum resources for a patient pinned in a vehicle: one Rescue, one Suppression unit, and one Battalion Chief.
- Consider the dispatch of additional units like Technical Rescue Team (TRT), Multiple Casualty Incident (MCI) level, Air Rescue, Heavy 1, Florida Power and Light (FPL), and Hazardous Material (Haz Mat) units.
Stabilization of Scene
- Implement crowd control with assistance from police and fire rescue personnel.
- Assign a Safety Officer promptly.
- Ensure personnel in the action area are in full turnout gear.
- Deploy a 1-¾" jumpline; use foam if fuels are present.
- Mitigate hazards: downed wires, fluid leaks, hazardous cargo, and fire.
Vehicle Stabilization
- Chock wheels as rescuers approach; place vehicle in “park” and turn off the engine.
- Disconnect the battery with caution, especially when electric seats may need movement.
- Stabilize the vehicle using cribbing at a minimum of three points and deflate the tires.
- Control breaking of glass using center punch or axe; clear shards with hand tools.
- Identify un-deployed airbags based on specific vehicle indicators and treat all vehicles as potentially having airbags.
Airbag Safety Protocol
- Disconnect the airbag power system, starting with the negative battery cable.
- Avoid placing personnel or equipment between airbags and the patient.
- Do not drill or cut into airbag modules to prevent accidental deployment.
- Exercise caution with hybrid vehicles; avoid areas near high voltage components, typically marked in bright orange.
Patient Stabilization
- Firefighter in protective gear gains access to the vehicle and reassures the patient while assessing and treating them inside.
Access and Disentanglement
- Access should be performed progressively, using hands, hand tools, and hydraulic tools.
- Establish a Tool Staging Area; include Genesis hydraulic tools for standard setup.
- Place the power plant to facilitate easy hose access from both sides of the vehicle.
- Disentanglement methods: open/ remove doors, flap/remove the roof, roll/lift the dash, displace seats, perform barn door or third door maneuver, displace the steering column, and pedals as necessary.
Termination
- Conduct scene cleanup: secure equipment, prevent medical waste or biological hazards.
- Complete MDFR National Fire Incident Reporting System (NFIRS) report, including TRT report, before shift end.
