    Study Notes

    Urinary System Organs

    • The urinary system is composed of two kidneys, two ureters, a urinary bladder and a urethra.
    • Kidneys filter waste products from the blood and produce urine.
    • Ureters transport urine from the kidneys to the bladder.
    • The bladder stores urine.
    • The urethra conveys urine from the bladder to the outside of the body.

    Urinary System Functions

    • The urinary system filters nitrogenous waste, toxins, and ions from the blood.
    • It regulates the volume and chemical composition of the blood, maintaining water, salt, acid, and base balances.
    • It produces regulatory enzymes, including renin which regulates blood pressure and kidney function, and erythropoietin which stimulates red blood cell production.
    • The urinary system metabolizes Vitamin D into its active form.

    Kidney Position

    • Kidneys are situated on the posterior wall of the abdomen, on either side of the vertebral column.
    • They are retroperitoneal, meaning they lie behind the peritoneum.
    • They extend from the 12th thoracic to the 3rd lumbar vertebra.
    • The right kidney is lower than the left due to the presence of the right lobe of the liver.

    Kidney Coverings

    • The kidneys are covered by several layers
    • The renal capsule, a fibrous layer that protects against infection and supports internal structures.
    • Perirenal fat, a layer of fatty tissue that provides cushion and stability.
    • Renal fascia, a dense connective tissue layer that surrounds the kidneys.
    • Pararenal fat lies external to the renal fascia.

    Kidney Posterior Relations

    • The kidneys are related posteriorly to muscles and the diaphragm.
    • The right kidney is associated with the twelfth rib.
    • The left kidney is related to the eleventh and twelfth ribs.

    Kidney Anterior Relations (Right Kidney)

    • The right kidney is anterior to:
      • Right suprarenal gland
      • Liver
      • Descending part of the duodenum
      • Right colic flexure
      • Small intestine

    Kidney Anterior Relations (Left Kidney)

    • The left kidney is anterior to:
      • Left suprarenal gland
      • Stomach
      • Spleen
      • Pancreas
      • Left colic flexure
      • Descending colon
      • Jejunum

    Internal Kidney Anatomy

    • The kidney has an outer cortex and a central medulla.
    • The medulla contains renal pyramids, cone-shaped structures.
    • Renal columns, extensions of the cortex, separate the pyramids.
    • Minor calices receive urine from renal papilla and join to form major calices
    • Major calices merge into a renal pelvis.
    • The renal pelvis is the superior end of the ureter.

    Nephrons

    • Nephrons are the functional units of the kidney that form urine.
    • Each nephron consists of:
      • Renal corpuscle (glomerulus and Bowman's capsule)
      • Proximal convoluted tubule
      • Loop of Henle
      • Distal convoluted tubule
      • Collecting duct

    Kidney Blood Supply

    • The kidneys receive blood directly from the abdominal aorta via the renal arteries.
    • Blood is returned to the inferior vena cava via the renal veins.

    Ureters

    • Ureters are muscular tubes, about 25 cm long.
    • They are a continuation of the renal pelvis.
    • They transport urine from the kidneys to the urinary bladder.
    • They run behind the peritoneum, making them retroperitoneal.
    • The ureter has three parts:
      • Abdominal part
      • Pelvic part
      • Intramural part (passes through the bladder wall)

    Urinary Bladder

    • The urinary bladder is a muscular sac that stores and expels urine.
    • It lies directly superior to the prostate gland in males.
    • The bladder has a neck that connects to the urethra.
    • An empty bladder is entirely within the pelvis; a full bladder expands into the abdominal cavity.
    • In males, the bladder lies anterior to the rectum.
    • In females, the bladder lies anterior to the vagina and inferior to the uterus.

    Bladder Sphincters

    • The bladder has two sphincter muscles that control urine flow:
      • Internal urethral sphincter:
        • Smooth muscle.
        • Involuntary control
      • External urethral sphincter:
        • Skeletal muscle
        • Voluntary control

    Male Urethra

    • The male urethra is approximately 20 cm long and serves as a pathway for urine and semen.
    • It has four parts:
      • Preprostatic part: between the bladder neck and the base of the prostate gland.
      • Prostatic part: passes through the prostate gland.
      • Membranous part: below the prostatic part.
      • Penile part: passes through the penis.

    Female Urethra

    • The female urethra is approximately 4 cm long.
    • It lies anterior to the vaginal orifice.
