Apparatus Malfunction Notification Triggers
- The following apparatus malfunctions require the unit to be placed Out of Service:
- Water pump failure
- Aerial device failure
- Brakes not operating properly
- Severe air leak
- Inoperative lights:
- Headlights
- Brake Lights
- Tail Lights
- Emergency lights
- Coolant leak (until checked)
- Substantial oil leak (until checked) in:
- Engine
- Transmission
- Rear End
- Power Steering
- Hydraulic System
- Damaged tires
- Belts beginning to squeal
- Steering suddenly feeling "loose"
- Any item deemed a safety issue by crews
Notification Procedure
- Upon identifying an apparatus malfunction, the unit must be immediately placed Out of Service
- Verbal notification must be provided to an on-duty Battalion Chief
- A maintenance ticket must be submitted in Checkit
- The notified Battalion Chief must notify the Assistant Chief of Operations or Assistant Chief of Support Services
