Study Notes
Laboratory Diagnosis
- Obtaining smears every 6-8 hours may be necessary to diagnose malaria
- Examine blood after a few hours (usually 6 hours) to arrive at specific diagnosis of species
- P.vivax, P.ovale, P.malariae all have asexual stages and gametocytes in the peripheral blood
- P.falciparum infections only have ring form and gametocytes in the peripheral blood
- Gametocytes may be seen in 10 days after symptoms begin
- Other stages are found in the capillaries of internal organs
- Blood smears should be obtained to monitor the response to treatment
- Monitoring should be continued until there is clearance of parasitemia
Malarial Rapid Diagnostic Tests (RDTs)
- Detect Plasmodium-specific antigens with a finger prick
- Can distinguish between falciparum and non-falciparum but not among non-falciparum species
- Use of PCR has been introduced
Treatment
- Main use of antimalarial drugs:
- Protective (prophylactic) - used before the infection occurs or before it becomes evident
- Curative (therapeutic) - action on established infection
- Preventive - the deterrence of infection of mosquitoes with the use of gametocidal drugs to attack the gametocytes in the blood of the human host
