Study Notes
RSV Vaccine (Beyfortus/Nirsevimab)
- Vaccine is for Respiratory Syncytial Virus (RSV)
- Administration guidelines provided
- Follow 7 rights of administration: patient, time (age, RSV season), immunization, dosage (weight-based), route/needle/technique, site, documentation.
Injection Technique
- Use 22-25 gauge needle
- Needle length varies by age and body mass
- Newborns (0-28 days): 5/8" needle, anterolateral thigh
- Infants (1-12 months): 1" needle, anterolateral thigh
- Toddlers (1-2 years): 1-1/4" or 5/8"-1" needle, deltoid muscle.
- Alternative needle lengths may be used based on skin tension
- %" needle for toddlers/children/patients < 130 lbs in deltoid muscle
- 1" needle for thigh muscle for adults
- Average volumes for injection
- Deltoid (arm): 0.5 mL (range 0.5-2 mL)
- Anterolateral thigh: 1-4 mL (range 1-5 mL)
Co-administration
- Simultaneous administration with other age-appropriate vaccines is recommended
- CDC creating job aid to address volume issues related to multiple injections.
Patient Risk Factors for Nirsevimab
-
Age: Infants under 8 months are not recommended for the vaccine
- Infants 8-19 month who meet high-risk criteria are recommended.
-
High Risk Criteria (8-19 months):
- Chronic lung disease of prematurity (requiring medical support) in the 6 months prior to the RSV season.
- Severe immunocompromised conditions
- Cystic fibrosis with severe lung disease (hospitalization, imaging abnormalities) or weight-for-length <10th percentile
- American Indian and Alaska Native children (for second-season prophylaxis)
-
Other Considerations:
- Maternal RSV vaccine during pregnancy
- Infant born within 14 days of maternal vaccination
- Previous dose of nirsevimab in the RSV season
- Consideration of nirsevimab when the potential benefit outweighs the risks for certain rare situations
- Patients should be weighed prior to administration. Dosages vary according to weight.
Other Vaccine Information
- Include age-appropriate combination vaccines (Pentacel, Pediarix, Vaxelis).
- Alternate routes (subcutaneous) are an option for some vaccines (IPV, MMR, Varicella, PPSV23)
- Take advantage of recommended age ranges for other vaccines (e.g., 3rd HepB dose at 18 months)
