Study Notes
Obstructive Sleep Apnea (OSA)
- Regular dental visits are recommended for patients with OSA to assess dental and periodontal condition and for fabrication and maintenance of oral appliances.
Pulmonary Tuberculosis (PTB)
- A contagious bacterial infection caused by Mycobacterium tuberculosis that involves the lungs and may spread to other organs.
- Diagnostics: Xpert MTB/RIF test or sputum smear microscopy.
- Treatment: Standard combination of four antituberculosis drugs (isoniazid, rifampicin, pyrazinamide, ethambutol) for 6 months, with two phases (initial and continuation).
- Dental guidelines: Elective dental treatment should be deferred until the patient has been declared noninfectious by a physician.
Sinusitis
- Dental guidelines: Consider using nasal decongestants or saline sprays before dental procedure to alleviate sinus congestion and facilitate easier breathing.
- Post-procedure care: Avoid activities that could aggravate symptoms and disrupt healing.
Streptococcal Throat Infection
- A contagious bacterial infection in the throat and tonsils caused by Group A Streptococcus.
- Most common in children, ages 5-15 years.
- Most dreaded complications: rheumatic fever and post-streptococcal glomerulonephritis.
- Symptoms: fever, pain when swallowing, sore throat, red and swollen tonsils, white patches or streaks of pus, tiny red spots on the roof of the mouth, and swollen lymph nodes.
- Goals of management: reduce symptoms, prevent acute and delayed complications, and prevent spreading of infection.
- Treatment: antibiotics (penicillin/amoxicillin), over-the-counter pain relievers (ibuprofen/acetaminophen), rest, and hydration.
- Dental guidelines: delay and reschedule dental procedures until after the infection has been successfully treated, and advise patients to complete the full course of antibiotics and practice good hygiene to prevent spreading of infection.
Asthma
- Characterized by episodic airway obstruction and airway hyperresponsiveness, usually accompanied by airway inflammation.
- Presents with episodic shortness of breath, wheezing, and cough, which can occur in relation to triggers or spontaneously.
- Hallmark: airway hyperresponsiveness.
- Most common complaint: episodes of shortness of breath, wheezing, chest tightness, mucus production, and cough upon exposure to triggers.
