Bony vs. Soft Tissue Landmarks

  • Bony Landmarks: Determining some bony landmarks can be challenging, while others are easily palpable. They stay fixed in place. Measurements based on bony landmarks can be replicated accurately.
  • Soft Tissue Landmarks: Easily identified, their positions change according to their mobility.

Extra-Oral Landmarks

  • Vermillion Border: The transitional epithelium between lip skin and mucous membrane. The amount depends on the bulk of the orbicularis oris muscle, the amount of labial alveolar bone, and the alignment of anterior teeth.
  • Philtrum of the Lip: A diamond-shaped depression at the center of the upper lip and the base of the nose. After tooth loss and alveolar bone resorption it may flatten.
  • Nasolabial Sulcus: A depression extending laterally and downwards from the ala of the nose. More prominent with aging due to tooth loss and loss of vertical dimension. Plumpers (thick denture flanges) can improve the sulcus but can sometimes interfere with muscular activity.
  • Mentolabial sulcus: This is a depression that runs horizontally between the lower lip and chin.
  • Ala-tragus line (Camper's line): An imaginary line from the superior border of the tragus of the ear to the inferior border of the ala of the nose; used to establish the posterior occlusal plane of artificial teeth
  • Inter-pupillary line: The imaginary line joining the pupils of the eye. It is used to establish the anterior occlusal plane of artificial teeth.
  • The angle of the mouth and the outer canthus of the eye: The distance between these is used to determine the vertical dimension of a patient at rest, useful for prosthetic purposes.

Modiolus

  • Located where the buccinator and other facial muscles meet, distal to the angle of the mouth. It can appear to drop with tooth loss. Positioning of maxillary teeth can improve this appearance.
