Pathophysiology of Nausea and Vomiting
- Nausea is stimulated by the vomiting center in the medulla of the brain
- Vomiting requires synchronized movements of the thorax and abdominal wall, gut, pharynx, and muscles of the mouth and face
- Multiple sources can trigger nausea and vomiting, including the GI tract, vestibular system of the ear, CNS centers, chemoreceptors outside the blood-brain barrier, myocardial infarction, heart failure, and increased intracranial pressure
- Anorexia often precedes nausea, and nausea frequently precedes vomiting
Diagnostic Tests for Nausea and Vomiting
- Nausea is often accompanied by pallor, sweating, tachycardia, and increased salivation
- Diagnostic tests include serum electrolytes tests, vagus nerve stimulation, CT or MRI of the head, pregnancy tests, liver, pancreatic, and renal function studies, imaging studies, and upper endoscopy
Interprofessional Care of the Patient with Nausea and Vomiting
- Most cases of nausea and vomiting are self-limited and require no treatment
- Pharmacological therapies include serotonin receptor antagonists, dopamine antagonists, antihistamines, dronabinol, nabilone, methylprednisolone, dexamethasone, lorazepam, and neurokinin receptor antagonists
- Postoperative nausea and vomiting (PONV) can delay healing and postpone discharge
