Questions and Answers

What is the description of Grade 1 power in the neurological examination?

  • Active horizontal movement with gravity eliminated
  • Active horizontal movement against gravity
  • Flicker of movement (correct)
  • No contraction
  • In the neurological examination, what does Grade 2 tone signify?

  • Affected part(s) rigid in flexion and extension
  • Considerable increase in muscle tone, making passive movement difficult
  • Slight increase in muscle tone manifested by a catch and release (correct)
  • Marked increase in muscle tone throughout the range of motion
  • What is the description of Grade 0 reflexes in the neurological examination?

  • Absence of reflexes (Areflexia) (correct)
  • Contraction of the muscle with more than 1 joint movement
  • Muscle contraction without joint movement
  • Contraction of the muscle and joint movement
  • What does Grade 3 power indicate in the neurological examination?

    Study Notes

    Neurological Examination

    Grading Power

    • Grade 0: No contraction
    • Grade 1: Flicker of movement
    • Grade 2: Active horizontal movement, with gravity eliminated
    • Grade 3: Active horizontal movement against gravity
    • Grade 4: Active movement against gravity and minimal resistance, which the examiner can overcome
    • Grade 5: Normal power

    Grading Tone

    • Grade 0: No increase in muscle tone
    • Grade 1: Slight increase in muscle tone, manifested by a catch and release or minimal resistance at the end of the range of motion
    • Grade 1+: Slight increase in muscle tone, manifested by a catch, followed by minimal resistance throughout the remainder of the range of motion
    • Grade 2: Marked increase in muscle tone throughout the range of motion, but affected part(s) are easily moved
    • Grade 3: Considerable increase in muscle tone, passive movement is difficult
    • Grade 4: Affected part(s) rigid in flexion and extension

    Grading Reflexes

    • Grade 0: Absent (Areflexia)
    • Grade 1: Reduced (Hyporeflexia), contraction of the muscle with no movement of the joint
    • Grade 2: Normal, contraction of the muscle and movement of the joint
    • Grade 3: Increased (Hyperreflexia), contraction of the muscle and movement of more than 1 joint
    • Clonus: Jerking the wrist/ankle back will give 5 or more flaps
