Grading Power
- Grade 0: No contraction
- Grade 1: Flicker of movement
- Grade 2: Active horizontal movement, with gravity eliminated
- Grade 3: Active horizontal movement against gravity
- Grade 4: Active movement against gravity and minimal resistance, which the examiner can overcome
- Grade 5: Normal power
Grading Tone
- Grade 0: No increase in muscle tone
- Grade 1: Slight increase in muscle tone, manifested by a catch and release or minimal resistance at the end of the range of motion
- Grade 1+: Slight increase in muscle tone, manifested by a catch, followed by minimal resistance throughout the remainder of the range of motion
- Grade 2: Marked increase in muscle tone throughout the range of motion, but affected part(s) are easily moved
- Grade 3: Considerable increase in muscle tone, passive movement is difficult
- Grade 4: Affected part(s) rigid in flexion and extension
Grading Reflexes
- Grade 0: Absent (Areflexia)
- Grade 1: Reduced (Hyporeflexia), contraction of the muscle with no movement of the joint
- Grade 2: Normal, contraction of the muscle and movement of the joint
- Grade 3: Increased (Hyperreflexia), contraction of the muscle and movement of more than 1 joint
- Clonus: Jerking the wrist/ankle back will give 5 or more flaps
