Study Notes
Metabolic Syndrome
- Metabolic syndrome is a cluster of conditions that increase the risk of cardiovascular disease, diabetes, and stroke.
- Key features include insulin resistance, visceral adiposity, atherogenic dyslipidemia, and endothelial dysfunction.
- These components are interconnected, sharing common mediators, pathways, and pathophysiological mechanisms.
- Visceral adiposity is a primary trigger for many of these pathways.
- It leads to insulin resistance, neurohormonal activation, and chronic inflammation.
- Neurohormonal activation involves the release of proteins and hormones like leptin, reactive oxygen species, and activation of the renin-angiotensin-aldosterone system.
- These processes contribute to oxidative stress, endothelial dysfunction, and platelet aggregation.
- Chronic inflammation is characterized by elevated fibrinogen activity and C-reactive protein (CRP).
- Atherosclerosis is a chronic inflammatory process beginning early, possibly even in the womb.
- Post-mortem studies link fatty streaks in blood vessels to maternal cholesterol levels.
- Symptoms of atherosclerosis often don't appear until later in life.
- Vascular inflammation is influenced by oxidative stress, leading to endothelial dysfunction and increasing the risk of plaque formation.
- Symptoms of atherosclerosis are dependent on CVD risk factors and individual genetics.
- Health behaviour interventions (like exercise) and pharmacological therapies can protect against atherosclerosis and even reverse plaque formation.
Diagnostic Criteria
- Diagnosed if three or more of these factors are present (or if medication is used to manage them):
- Increased waist circumference (visceral fat)
- High blood pressure
- High blood sugar
- High triglycerides
- Low HDL cholesterol (good cholesterol)
- The National Cholesterol Education Program (NCEP) Adult Treatment Panel III (ATP III) definition is commonly used.
Prevalence
- 1 in 5 Canadian adults (aged 18-79) have metabolic syndrome.
- Risk increases with age.
- About 1 in 10 Canadians aged 18-39 have the syndrome.
- Prevalence is higher in individuals aged 60-70.
- In 2021, 29% of Canadian adults were obese and 36% were overweight.
Causes and Implications
- Rising prevalence globally.
- Public health concern over lifestyle modifications needed to decrease obesity and increase physical activity..
- Increased clinical concern regarding the need for screening and early identification for timely treatment and risk reduction.
- Increase in obesity and sedentary behaviour are contributing factors in Canada.
- Lack of sufficient physical activity is contributing to the development of the syndrome in Canada.

Description
Overview of metabolic syndrome, a cluster of conditions increasing cardiovascular disease, diabetes, and stroke risk. Key features include insulin resistance, visceral adiposity and endothelial dysfunction. Visceral adiposity triggers pathways leading to insulin resistance, neurohormonal activation, and chronic inflammation.
