Membranous Layer of Superficial Fascia and Extravasation of Urine
- The membranous layer of the superficial fascia is an important clinical structure.
- It forms a potential closed space that does not open into the thigh.
- This space is continuous with the superficial perineal pouch via the penis and scrotum.
- Rupture of the penile urethra can lead to extravasation of urine.
- Urine can spread into the scrotum, perineum, penis, and even into the lower part of the anterior abdominal wall deep to the membranous layer of fascia.
Membranous Layer of Superficial Fascia and Abdominal Wounds
- The membranous layer of the superficial fascia is attached to the fascia lata.
- This attachment prevents the spread of urine into the thigh.
- Surgeons usually suture the divided membranous layer during abdominal wound closure.
- It strengthens the healing process, prevents skin scar stretching, and improves cosmetic outcome.
Anterior Abdominal Wall: Deep Fascia
- The deep fascia in the anterior abdominal wall is a thin layer of connective tissue covering the muscles.
- It lies immediately deep to the membranous layer of superficial fascia.
Anterior Abdominal Wall: Muscles
- The anterior abdominal wall consists of 5 muscles:
- External oblique
- Internal oblique
- Transversus abdominis
- Rectus abdominis
- Pyramidalis
- These muscles are arranged in three broad anterolateral layers and two midline flat muscles.
- They are a continuation of the thorax muscles, specifically the external, internal, and innermost intercostal muscles.
- The neurovascular plane lies between the internal oblique and transversus abdominis muscles.
Anterior Abdominal Wall: External Oblique Muscle
- The external oblique is a broad, thin, muscular sheet known as the "pocket muscle."
- It originates from the outer surfaces of the lower 8 ribs.
- It inserts via a broad aponeurosis into the:
- Xiphoid process
- Linea alba
- Pubic crest
- Pubic tubercle
- The superficial inguinal ring is a V-shaped defect in the external oblique aponeurosis located immediately above and medial to the pubic tubercle.
- The lower border of the aponeurosis folds backward on itself, forming the inguinal ligament ("Poupart ligament") between the anterior superior iliac spine and the pubic tubercle.
