Independent Level of Assistance
- Patient completes activities consistently and safely without any physical or verbal help.
Independent with Equipment Level of Assistance
- Patient performs all tasks independently after the necessary equipment has been supplied.
Independent with Set-Up Level of Assistance
- Patient can complete activities consistently and safely once the setup is arranged, with or without equipment involved.
Supervision Level of Assistance
- Patient carries out activities but needs verbal prompts and/or intermittent observation to address infrequent issues.
Close Supervision Level of Assistance
- Patient performs tasks with continuous verbal prompts and requires ongoing observation for safety and consistency.
Contact Guard Level of Assistance
- Patient can finish tasks but necessitates the therapist’s hands-on presence for safety, without direct physical assistance.
Minimal Assist Level of Assistance
- Patient performs around 75% of tasks independently, with 25% of the activity requiring physical help from the therapist.
Moderate Assist Level of Assistance
- Patient manages approximately 50% of the activity independently, needing equal physical assistance (50%) from the therapist.
Maximal Assist Level of Assistance
- Patient is capable of completing 25% or less of the required tasks, with 75% or more physical support from the therapist.
Dependent Level of Assistance
- Patient is entirely unable to perform any aspect of the activity and requires complete assistance.
