Master blind list that prepared by area staff of operations, Approved by Area Owner for listing and identifying all blinds and pipe disconnection. 56 Classification: Tasnee - General Information Safe Making Process Isolation Process Isolation including the following categories: Positive Isolation Process Isolated by Installing Rated Blind or Disconnecting Pipe. Considered as process isolation for Hot Work and/or Confide Space Entry. To verify that the isolation valves are holding, Bleeder must show no leakage. All valves shall be locked and tagged with a “______ - Do Not Operate” tag.

Process Isolation including the following categories: Proved Isolation Process Isolated by Double Block & Bleed method is Considered as process isolation for Cold Work only. Double Block & Bleed method is closing of two positives shut off valves and opening a bleed (vent/drain) valve in between. The theory behind this method is that if the first isolation valve fails, the material in the line would discharge through the bleed valve and not into the work area. In some cases, valves used for a double block and bleed may be remotely or automatically operated. In some cases, the energy supply (air, electricity, etc.) shall be disconnected and the position of the valve shall be assured by a spring, pin or other locking device.

Study Notes
Oxygen-Rich Atmospheres
- Expand the range of flammable materials
- Lower the ignition energy
- Increase the rate of combustion when materials are ignited
Work Permits
- General Managers must be certified in work permit and countersign permits for activities like hot taps, work on flare headers, and Category A confined space entry
- Area Owners must be certified in work permit and countersign permits for activities like hot work, radioactive source work, and confined space entry
- Area Owners must ensure permit issuers, receivers, safe makers, gas testers, and standby persons are trained and competent
Safe Making Processes
- Ensure that equipment to be worked on has been positively identified and tagged
- Process Isolation includes Positive Isolation by means of Installing Rated Blind or Disconnecting Pipe for Hot Work and/or Confined Space Entry
- Proved Isolation by Double Block & Bleed isolation method is used for Cold work only
- Isolation process Isolated by Single Valve & Bleed for Cold Work in very limited cases
- Mechanical Isolation includes physical restraining movement due to weight, stored energy or effect of wind circulation even after power is isolated for Cold work only
- Some methods are used disconnecting belts, chain drives and mechanical links on mechanically driven equipment’s
Isolation Process Requirements
- Any isolated equipment/system shall contain all and/or the following:
- Tag list that is prepared and verified by shift supervisor
- Master blind list that prepared by area staff of operations, Approved by Area Owner for listing and identifying all blinds and pipe disconnection.
Isolation Process Categories
-
Positive Isolation: Process Isolated by Installing Rated Blind or Disconnecting Pipe. Considered as process isolation for Hot Work and/or Confined Space Entry.
- To verify the isolation valves are holding, the Bleeder must show no leakage.
- All valves shall be locked and tagged with a “Danger - Do Not Operate” tag.
-
Proved Isolation: Process Isolated by Double Block & Bleed method is Considered as process isolation for Cold Work only.
- The theory behind this method is that if the first isolation valve fails, the material in the line would discharge through the bleed valve and not into the work area.
- In some cases, valves used for a double block and bleed may be remotely or automatically operated.
- In some cases, the energy supply (air, electricity, etc.) shall be disconnected and the position of the valve shall be assured by a spring, pin or other locking device.
Safety Responsibilities
-
Permit Issuer/Receiver:
- Ensure all hazard mitigations in the permit are followed and are alerted to changing hazards
- Be competent to use the safety equipment
- Present themselves to the permit issuer with their authorization card, before signing on the work permit
- Ensure that JSA briefing is carried out before and after taking the signature specially for the employees who did not attend JSA meeting
- Ensure to upload photos of JSA into electronic system
-
Area Owner:
- Ensure that permit issuers, receivers, safe makers, gas testers, and standby persons are trained and competent
- Ensure the attendance sheet on the JSA form is signed by all craftsmen and supervisors carrying out the task
- Ensure JSA is reviewed and approved
- Ensure that he 'signs in' or 'signs out' as appropriate on (the Confined Space Entry Log)
-
Safe Maker:
- Ensure that equipment to be worked on has been positively identified and tagged
JSA (Job Safety Analysis) Review
- Review Task Steps, Description of Hazard, Control Measures Required Responsible Person, Signature after Implementation in JSA form
- Review JSA
Safety Measures and Aims
- Gas Testers: Verify the gas content of the space before entry.
Hot Work
- Initial test: Flammability testing should be done before commencing hot work
- Flammable and Combustible Materials: Flammable and combustible materials should be controlled around the hot work area by removing or isolating them
- Gas Cylinders and Welding Machines: During welding/cutting operations, gas cylinders and welding machines should be placed in a safe location, protected from extreme temperatures, and secured to prevent tipping over.
- Oxygen Cylinder Hoses and Fittings: Oxygen cylinder hoses and fittings should be handled with care, avoiding kinks, damage, or contact with oil and grease.
- Valves on Cylinders and Torches: When not in use, valves on cylinders and torches should be closed tightly to prevent accidental leakage.
- Gas Cylinders Storage: During welding/cutting operations, gas cylinders should be stored at least 20 feet away from the work area, in a well-ventilated area, and protected from direct sunlight and extreme temperatures.
Confined Space Entry
- Confined Space Entrant: Exit and alert the Standby man if they detect a change in hazardous conditions
- Standby/ Fire Watch: Ensure emergency equipment is ready for use
- JSA Form: The third column of a JSA form should list the control measures to eliminate or mitigate hazards identified
- JSA Form: The fourth column of a JSA form is responsible for the recommendations listed in the fourth column of a JSA form
- Control Measures: To eliminate or mitigate hazards, use the Hierarchy of Controls approach.
