  • Each specimen is assigned a unique accession number once specimen identity is established.
  • The accession number is used to verify and maintain specimen identity throughout the entire process.
  • Specimens can be computer accessed in CoPathPlus or manually accessed during periods of CoPathPlus and/or MHS GENESIS downtime.
  • Reagents and supplies needed for the procedure are a computer with MHS GENESIS and CoPathPlus access, accession labels and printer, and office supplies.
  • The procedure is as follows:
  • Log into CoPathPlus IAW AP-HIS.211.
  • Enter patient information in the patient database as indicated in AP-HIS.A215.
  • Generate labels for requisitions and specimen containers with the assigned accession number (i.e., BAS18-001).
  • Affix label on patient specimen(s) and requisition after comparing printed label to specimen(s) and requisition label/documentation.
  • Compare the following prior to labeling:
  • Patient Specimen: Patient accession number, patient last name, first name (first 14 characters), specimen site.
  • Requisition/Working Draft: Patient accession number, patient last name, first name (first 14 characters), DODID, Accession Date, DOB.
  • During CoPathPlus/MHS GENESIS short-term downtime, wait to see if the system comes back up.
  • During CoPathPlus/MHS GENESIS long-term downtime, manual accessioning is performed.
  • If both systems are down, all in-house specimens are submitted with SF 515s. For outside specimens, a copy of the specimen information from the shipping list is placed with the corresponding specimen container(s) in a plastic bag.
  • If only CoPathPlus is down, use SF 515s or order printouts from MHS GENESIS to include the list of specimens ordered and clinical history. Outside specimens follow the same procedures as if both systems are down.
  • When CoPathPlus is back up, the on-call technician will accession the cases in CoPathPlus. This must be done very carefully, with minimal distractions, to avoid errors.
  • Any tissue samples received must be accessioned in CoPathPlus.
  • CoPathPlus allows users to access histology case data in the system.
  • The system is updated to version 4.0, which includes new features for accesing histology case data.
  • Starting 10-Aug-2022, users will be able to access histology case data in CoPathPlus.
