Standard Operating Procedure for Health and Safety, Rehabilitation
- Purpose: To provide guidance on rehabilitation during emergencies and training, maintaining personnel safety. Ensures personnel do not deteriorate, jeopardizing the operation or safety of others. Improves performance through evaluation/rehabilitation for conditions like heat buildup, dehydration, physical exertion
Definitions
- Incident Management System (IMS): System defining roles and responsibilities for incident management, used synonymously with Incident Command System (ICS).
- Canteen: A support vehicle with food, water, ice, and beverages for personnel at incidents.
- Medical Monitoring: Ongoing evaluation of firefighters at risk of adverse effects from stress, heat/cold, or hazardous environments.
- Rehabilitation: Intervention to mitigate physical/physiological/emotional stress of firefighting/training. Sustains energy, improves performance, reduces on-scene or training injuries.
- Rehab: The area at an incident where personnel rehydrate, rest, or rehabilitate. May also be referred to as the Rehabilitation Unit/Group. Managed by a Rehab Unit/Group Leader or Supervisor.
- Rehab Vehicle: A designated vehicle for firefighter rehabilitation and rehydration, air-conditioned with seating for personnel. Stocked with water, ice, and beverages.
Rehabilitation Process
- Process: Includes rest, hydration, cooling (passive/active), warming, medical monitoring, emergency care, and stress relief.
- Large-Scale Incidents: Rehabilitation procedures established for large-scale, long-duration, and/or physically demanding incidents as well as extreme temperatures.
- Cooling Method: In hot environments, direct transition into cool/cold air conditioning can negatively impact cooling systems. Shade and fans are advised.
- Rehab Area: Designated and marked area for rehabilitation (fire line tape, or other marking). The area is typically two floors below the fire floor in high-rise situations.
Incident Commander Responsibilities
- Responsibilities: Understand need for rehabilitation during incidents/training. Establish Rehabilitation Unit/Group. Determine rehabilitation area location.
- Personnel monitoring: Officers responsible for member well-being and safety.
- Relief requests: Appropriate process for requesting relief provided by company officers.
- Crew integrity: Personnel will remain in rehab area until authorized by assigned rehab officers.
- Par tags: Par tags to be brought to rehab.
Personnel Considerations
- Hydration: During hot weather, encourage water consumption. Personnel should report fatigue/exposure risks to supervisor.
- Supervision: Maintaining awareness of member concerns regarding health and safety.
- Heat/Cold Stress: Understanding symptoms and reporting to the officer promptly.
- Rehab Requirements: Promptly report need for rehab to supervisor.
- Maintain unit integrity: Teams remain together throughout the incident if possible.
Rehabilitation Unit Leader/Supervisors
- Selection/Location: Leader/Supervisor is assigned/selected for appropriate rehab procedures.
- Logistics: The location is generally assigned by the Logistics Section leader.
- Rest: Areas should be free of exhaust fumes, hazardous atmospheres, and smoke.
- Resources: Adequate resources (EMS care) ensuring appropriate levels of care.
Medical Monitoring
- Early Identification: EMS personnel actively assess potential issues, especially heart rate, blood pressure, and oral temperatures.
- Criteria for immediate action: Heart rate over 110 beats/minute or oral temperatures over 100.4 degrees requires additional care.
- Heart rate sustained over 100: Firefighters sustaining a heart rate greater than 100 BPM after 20 minutes or more of rest should not be released from rehab and should remain in the section for monitoring.
- Blood pressure: Significant high blood pressures greater than 160 systolic or 100 diastolic require continuing monitoring and treatment and should consider medical facilities.
- Transport: Personnel experiencing significant problems (chest pain, difficulty breathing, nausea, and/or headache) should be immediately transported.
General Considerations
- Active Cooling: Use of mist fans and other methods is necessary for situations requiring active cooling during situations of prolonged exposure or high temperatures.
- Cooling duration: 10 minutes (minimum) to 20 (preferred) active cooling following each SCBA use, essential considerations especially in hot environments
Personnel during Rehabilitation
- Rest periods: Minimum of 10-20 minutes of rest before reassignment/release.
- Continued medical monitoring: On-going medical monitoring and support.
- Abnormal vital signs: Frequent monitoring required if abnormal vital signs are observed.
- Transport to hospitals: Transport if needed for serious medical issues.
- Hydration: Encourage water and sports drinks for hydration. Caffeine-free, carbonated beverages avoided.
