Clearwater Fire & Rescue Department Standard Operating Procedure - Radio Incident Scene Size-up Report
- Purpose: Define critical information needed for responding to structure fires, reported structure fires, unconfirmed structure fires, and fire alarms. Create a consistent and structured method for delivering initial incident information to all units.
- Size-up Report (625.1):
- First arriving unit provides radio size-up upon arrival at structure fires.
- If first arriving unit is not an engine company, the first engine company must announce water supply/layout information.
- Radio Size-up Report Information (625.2):
- Correct address (if different from dispatched address).
- Number of floors.
- Type of occupancy (e.g., single-family, apartment, commercial).
- Site conditions and pertinent information (e.g., smoke showing, fire showing, nothing showing, etc.).
- Water supply/layout information (e.g., hydrant location, lay type).
- Current mode of attack (offensive, defensive, investigating).
- Correct Address (625.3):
- Announce the correct address if the initial dispatch address is incorrect.
- Announce Number of Floors (625.4):
- Report the number of floors in the building. If unsure, state it is a multi-story structure, until confirmed.
- Announce Type of Occupancy (625.5):
- Convey the type of building (e.g., single-family home, apartment complex).
- Announce Conditions/Pertinent Information (625.6):
- Describe current scene conditions (e.g., smoke direction, fire location). Include significant observations/details (e.g., construction type).
- Announce Water Supply/Layout (625.7):
- Describe water supply details, hydrant information, water lay type (forward, reverse, split).
- Announce Mode of Attack (625.8):
- Report if the mode of attack is Offensive, Defensive or Investigating.
- Radio Report Example (625.9):
- Example: "Engine 51 is at 100 Main Street. We have a single-story residential structure with fire showing from two windows on the alpha side. Engine 51 will be in the Offensive mode." Given Example.
- Other Size-Up Reports (625.10):
- Other incidents (vehicle accidents, gas leaks, hazardous materials) follow different procedures and are covered by SOP 614.
