    Study Notes

    Entamoeba Histolytica

    • Entamoeba histolytica causes intestinal amebiasis, which leads to bloody diarrhea (red stool).

    Life Cycle

    • The parasite has two main life cycle stages:
      • Cyst form: ingested from contaminated water, and can be transmitted through anal-oral contact (men drinking water holding hands).
      • Liver shape: trophozoite form invades the liver, causing amoebic liver abscess, most commonly in the right lobe (liver shape dig site and hole in right side of liver).

    Clinical Features

    • Amoebic liver abscess causes right upper quadrant (RUQ) pain, with an enlarged and tender liver (map is a ct scan of liver and a man holding his right upper quadrant).
    • The abscess has an "anchovy" paste consistency (anchovy paste truck).
    • Intestinal amebiasis causes ulcerations along the colon (rust spots).
    • Flask-shaped ulcers can occur in the colon (Erlenmeyer flasks).

    Diagnosis

    • Stool O&P (puddle with floating red cups) is used to detect cysts or trophozoites, and will contain RBCs.
    • An ELISA antigen test or serology can be used for diagnosis (flask in colon).

    Treatment

    • Metronidazole is an effective treatment for Entamoeba histolytica (Metra).
    • Drugs that work in the lumen of the intestine, such as Paromycin (pair of mice near metro), are also used to treat the infection.
