Endometriosis
- Endometriosis is a condition where tissue similar to the lining of the uterus (endometrium) grows outside the uterus.
- It’s commonly found in women during their reproductive years (ages 25-35), though it can occur as young as 11 years old.
- While the exact cause is unknown, theories include:
- Retrograde menstruation: Menstrual blood flows back through fallopian tubes into the pelvic cavity, leading to endometrial cell implantation.
- Coelomic metaplasia: Primitive cells lining the pelvic organs develop into endometrial cells.
- Surgical transfer: Endometrial tissue is transferred during surgery, such as episiotomies or caesarean sections.
- Bloodstream/lymphatic system: Endometrial cells travel via bloodstream/lymph to distant organs (rare).
- Immune system alterations/genetics: Immune system fails to recognize and destroy misplaced endometrial tissue.
Symptoms
- The most common symptom is severe, recurrent pelvic pain.
- Pain is not related to the extent or stage of endometriosis.
- Women can have little/no pain despite extensive endometriosis or significant pain with minimal endometriosis.
- Other possible symptoms:
- Dysmenorrhea: Painful menstrual cramps, worsening over time.
- Chronic pelvic pain: Often accompanied by lower back and/or abdominal pain.
- Dyspareunia: Painful intercourse.
- Dysuria: Urinary urgency, frequency, and painful urination.
- Infertility: Endometriosis can lead to anatomical distortions and adhesions that impair conception.
- Gastrointestinal symptoms: May mimic irritable bowel syndrome.
- Nausea, vomiting, and/or diarrhea: Often experienced just before or during a period.
- Heavy or prolonged menstrual periods: Frequent menstruation or short menstrual cycles.
- Mood swings and fatigue:
- Acute abdomen: If an endometriotic cyst (chocolate cyst) ruptures, presenting as a medical emergency.
Diagnosis
- History and physical exam: Thorough assessment of pelvic pain and other symptoms.

- CBC: To check for anemia, inflammation, and infection.
- CA-125: Blood test that checks for a protein commonly elevated in ovarian cancer, but may also be elevated in endometriosis.
- Transvaginal ultrasound: To visualize pelvic anatomy and identify possible endometriomas.
- MRI: To assess cyst size and any adhesions.
- Diagnostic laparoscopy: Surgical procedure where a small camera is inserted into the abdomen, allowing for direct visualization of lesions, adhesions, and obtaining biopsy samples.

- Laparoscopy with biopsy: Necessary for a definitive diagnosis.
- Hysterectomy with biopsy: In rare cases, this can be done to diagnose adenomyosis (endometrial tissue within the uterine wall).
Treatment
