Study Notes

Acetaminophen

  • Acetaminophen, also known as APAP or Tylenol, is used by EMTs for pediatric seizures and to alleviate discomfort from severe injuries and fevers.
  • Pediatric dosage: 15 mg/kg.
  • Adult dosage (for patients ≥ 2 years): 10 mg/kg or 650 mg.
  • Administered as a rectal suppository for pediatric patients.
  • Medical control status for acetaminophen is noted as a standing order.

Oxygen

  • Oxygen is utilized by EMTs for airway management and respiratory support, having no contraindications in prehospital settings.
  • Administer at 15 liters per minute (lpm) for both adult and pediatric patients using non-rebreather or bag valve mask.
  • For nasal cannula, oxygen flow rate ranges from 1-6 lpm.
  • Medical control status for oxygen is established as a standing order.

Epinephrine

  • Epinephrine is indicated for conditions such as anaphylaxis, asthma, dyspnea, and burns; it requires medical control for patients with cardiac history or those over 50 years old.
  • Adult intramuscular dosage: 0.3 mL of 1:1000 solution.
  • Pediatric dosing varies:
    • For children over 20 kg: 0.3 mL of 1:1000.
    • For children 10-20 kg: 0.2 mL of 1:1000.
    • For children under 10 kg: 0.1 mL of 1:1000.
  • Epinephrine can also be administered intranasally and via nebulization (5 mL of 1:1000 solution) with medical control contact required in RI.

Oral Glucose

  • Oral glucose is indicated for patients with impaired consciousness, seizures/postictal states, or signs/symptoms of hypoglycemia (blood glucose < 60).
  • Contraindications involve airway concerns, nausea/vomiting, or absence of gag reflex.
  • Dosage for both adults and pediatric patients: 15 g.
  • Medical control for oral glucose administration is established as a standing order.

Glucagon

  • Glucagon is indicated for patients with impaired consciousness, seizures, or hypoglycemia, and has no contraindications in emergencies.
  • Route of administration is intramuscular.
  • Adult dosage: 1 mg if blood glucose is < 60 or unknown.
  • Pediatric dosage: 0.1 mg/kg, up to a maximum of 1 mg.
  • Requires contact with medical control.

Naloxone

  • Naloxone, known as Narcan, is indicated for cases of impaired consciousness or seizures; caution is advised for known addicts.
  • Dosage in suspected opioid overdose: 2 mg, repeat until level of consciousness improves or maximum 10 mg.
  • If overdose is not suspected, dosage: 0.4 mg, repeat every minute until level of consciousness improves or maximum of 2 mg.
  • Medical control must be contacted when administered intramuscularly or for suspected overdose, or if more than 0.4 mg is required.
  • Standing order exists for intranasal administration, limited to an initial 0.4 mg dose.
