Contraindications to Drug Assisted Airway Placement
- The most important contraindication to drug-assisted airway placement is the inability to insert a supraglottic airway device (SGA) or endotracheal tube (ETT).
- Relative contraindications include:
- Inability to adequately visualize or impending airway obstruction.
- This includes conditions such as burns, anaphylaxis, epiglottitis, retropharyngeal abscess, partial foreign body obstruction, neck trauma, and angioedema.
- Any patient with a partial airway obstruction may progress to a complete airway obstruction due to the loss of muscle tone caused by paralytics.
- Mandatory online medical control (OLMC) is required if an impending airway obstruction is identified.
- Consult with OLMC, an ETT may be the preferred option in an impending airway obstruction.
- Adequate ventilation and oxygenation achieved with basic airway management.
- Basic airway management techniques are often effective and lower risk than advanced airway techniques in the prehospital setting.
- Patients critically hypoxic despite basic airway management should be assessed for difficulty with airway intervention.
- Mandatory online medical control (OLMC) is required to discuss facilitation of ETT and/or advanced airway adjuncts with medications.
