Study Notes
Digestive System
- The digestive system breaks down food, absorbs nutrients, and eliminates waste.
- Anatomy: Mouth, esophagus, stomach, small intestine (duodenum, jejunum, ileum), large intestine, rectum, anus.
- Accessory organs: Liver (produces bile), gallbladder (stores bile), pancreas (digestive enzymes & insulin)
Common Gastrointestinal Disorders
- Functional disorders: Affect digestive system function without a specific cause, e.g., Irritable Bowel Syndrome (IBS)
- Inflammatory disorders: Involve inflammation of the digestive tract, e.g., Inflammatory Bowel Diseases (IBD)
- Structural disorders: Physical changes in the digestive system, e.g., ulcers, diverticulitis, tumors
- Malabsorption disorders: Inability to absorb nutrients, e.g., Celiac disease, lactose intolerance
Irritable Bowel Syndrome (IBS)
- Functional gastrointestinal disorder causing abdominal pain, bloating, and altered bowel habits.
- Symptoms vary and fluctuate in severity.
- Diagnosis: Patient history, stool tests, colonoscopy, blood tests, exclusion of red-flag symptoms.
- Treatment: Laxatives, antidiarrheal agents, antispasmodics, antidepressants, probiotics, cognitive-behavioral therapy (CBT), stress management.
- Nursing Interventions: Monitor weight, dietary restrictions, small meals, pain management, emotional support
Inflammatory Bowel Disease (IBD)
- Chronic inflammation of the gastrointestinal tract, includes Crohn's disease and ulcerative colitis.
- Crohn's disease: Affects any part of the digestive tract, causes inflammation in patches, may lead to narrowing or thickening of the affected area.
- Ulcerative colitis: Primarily affects the colon, causes inflammation and ulcers in the lining of the colon, leading to diarrhea, abdominal pain, and rectal bleeding.
- Symptoms: Abdominal pain, diarrhea, rectal bleeding, weight loss, fatigue, fever.
- Diagnosis: Medical history, physical exam, blood tests, imaging studies, endoscopy with biopsy.
- Nursing Diagnoses: Acute pain, diarrhea, imbalanced nutrition, risk for impaired skin integrity, disturbed sleep pattern, risk for infection, risk for deficient fluid volume.
Gastroesophageal Reflux Disease (GERD)
- Chronic condition where stomach acid flows back into the esophagus, causing heartburn, regurgitation, chest pain, and difficulty swallowing.
- Diagnosis: Clinical history, upper GI endoscopy, esophageal pH monitoring, esophageal manometry, barium swallow.
- Nursing Diagnoses: Acute pain, impaired swallowing, risk for aspiration, imbalanced nutrition, deficient knowledge, risk for impaired skin integrity, disturbed sleep pattern.
Peptic Ulcers
- Sores on the inner lining of the stomach, upper small intestine, or esophagus caused by stomach acid erosion.
- Causes: Helicobacter pylori infection, Nonsteroidal Anti-inflammatory Drugs (NSAIDs), excessive stomach acid, smoking, alcohol consumption.
- Diagnosis: Medical history, endoscopy, H.pylori testing, upper GI series.
- Nursing Diagnoses: Acute pain, imbalanced nutrition, risk for deficient fluid volume, anxiety, knowledge deficit, risk for ineffective coping.
Gallstones
- Solid particles in the gallbladder, causing blockages in bile ducts, resulting in sharp pain, nausea, and potential infection.
Celiac Disease
- Autoimmune disorder where ingesting gluten damages the small intestine, leading to malabsorption, weight loss, and fatigue.
Diverticulitis
- Inflammation of irregular bulging pouches in the wall of the large intestine, causing pain, fever, and digestive disturbances.
Hepatitis
- Inflammation of the liver caused by viruses, alcohol, or other factors, leading to liver damage, jaundice, and chronic liver disease.
Complications of GIT Disease
- Gastrointestinal bleeding
- Perforation
- Obstruction
- Peritonitis
- Malnutrition
- Dehydration
- Chronic Liver Disease
- Anemia
- Cancer
- Sepsis
