Disease Framework vs. Illness Framework
- The disease framework is focused on the doctor's agenda, while the illness framework is focused on the patient's agenda.
- Patient-centered care involves providing care that is respectful of, and responsive to, individual patient preferences, needs, and values.
Components of Patient-Centered Care
- Elicit the patient's agenda
- Avoid starting with "How are you feeling?" or "How are you today?" as it may lead to somatization of concerns into physical symptoms.
- Use open-ended questions to discuss the patient's agenda items, such as "How may I help you today?" or "What can I do for you today?"
- Ask about the patient's ideas, concerns, expectations, feelings, and the impact of the problem on their daily life.
Eliciting the Patient's Agenda
- Ask about the patient's ideas: "What do you think is the cause of…?" or "Do you have any thoughts on what might be causing this?"
- Ask about the patient's concerns: "What do you worry about regarding your health?" or "Is there something you worry might happen?"
