Chronic Kidney Disease (CKD) Staging

CKD is classified by severity into five stages, from 1 to 5.
Stage 2 of CKD is coded as N18.2, indicating mild chronic kidney disease.
End Stage Renal Disease (ESRD) is coded as N18.6 and should be assigned when documented by the provider.

Coding for CKD and ESRD

When a provider documents a stage of CKD alongside ESRD, only code N18.6 for ESRD should be used.
For patients with both CKD and a history of kidney transplantation, use the appropriate N18 code for CKD, along with Z94.0 to indicate kidney transplant status.

Hematuria Coding

If a physician notes that a patient’s hematuria is recurrent, persistent, or idiopathic, it should be coded under category N02 for recurrent and persistent hematuria.

Robotic Assisted Surgery Coding

Procedures performed using robotic assistance require an additional code.
