    Study Notes

    Stages of Treatment for Caries

    • Stage 1: Acute Treatment involves excavating open cavities, placing temporary cement, and performing necessary extractions and emergency endodontic treatments.

    • Stage 2: Management of Caries Disease focuses on diet history and counseling. Salivary and bacteriological tests are performed, along with oral hygiene instruction and plaque control, and topical fluoride application. Fissure sealing is also included.

    • Stage 3: Restorative (Operative) Treatment

    • Stage 4: Risk Assessment and Follow-Up Program

    Choice of Restoration

    • The type of restoration for primary teeth depends on the tooth needing restoration, the past caries history and cooperation of the child.

    Restorative Procedures

    • Conventional methods, including extension for prevention, drill and fill, and the use of G.V. Black's techniques are described.

    • Modern techniques that are tooth substance saving include pit and fissure preparations, box-only preparations, and adhesive cavities, using lasers and air abrasion. Less (or no) anesthesia may be used.

    Primary Teeth Characteristics

    • Primary teeth have thinner enamel and dentin.
    • Primary tooth pulp chambers are larger relative to crown size compared to permanent teeth.
    • Primary tooth pulp horns are closer to the outer tooth surface than in permanent teeth.
    • Enamel rods of primary teeth extend occlusally from the dentin-enamel junction, in contrast to permanent teeth where they extend cervically.
    • Primary teeth demonstrate greater cervical constriction and broader proximal surfaces compared to permanent teeth.
    • Primary teeth are whiter than their permanent successors.

    Caries Excavation

    • Complete removal (non-selective) of carious tissue is one approach
    • Selective removal of carious tissue prioritizes different degrees of removal for peripheral and pulp areas, based on depth.
    • Partial and incomplete removal are also options, as well as ultra-conservative removal.
    • No caries removal is also a possibility.
    • Hand instruments and rotary instruments can be used.

    Handpieces

    • Low speed handpieces (500 to 15,000 rpm) are commonly used for caries removal and polishing.
    • Light pressure and brushing strokes are important while using low-speed handpieces to avoid damage.
    • High-speed handpieces (100,000 to 300,000 rpm) should be combined with coolant, and intermittent cutting using light pressure is recommended to prevent excessive heat.

    Dental Instruments

    • Different types of dental burs are available, with variations in shape, size and material for specific tasks. Differences are noted in shank types, including latch-type and straight designs.

    Tooth Restoration Techniques

    • Adhesive restorations require precise application and overall clinical performance is strongly dependent on technique, patient selection, and a skilled operative.
    • Adhesive materials have less forgiveness in handling and are more technique sensitive compared to amalgam restorations.
    • Options for restorative materials include composites, compomers, resin-modified glass ionomers, and glass ionomers, and stainless steel crowns.
