ADPIE Process
- ADPIE is a process that helps medical professionals remember the order of the 5 sequential steps of the nursing process
- The 5 steps of the nursing process are: Assessment, Diagnosis, Planning, Implementation, and Evaluation
Assessment
- The first step of the ADPIE process, collecting and recording data
- There are 2 types of collecting data in assessment:
- Subjective data (cannot be measured or observed directly, includes verbal and asking questions)
- Objective data (can be measured, often referred to as signs, can be seen, heard, felt, or smelled)
Diagnosis
- The process of developing a theory about an individual's situation
Planning
- The process of developing a plan and establishing SMART goals to achieve a desired outcome
- SMART goals are:
- Specific (clear, precise, and specific)
- Measurable (quantifiable and measurable, involves using concrete criteria to track and evaluate progress)
- Attainable (realistic and attainable within the given resources)
- Relevant (aligns with broader objectives and is relevant to the individual's needs)
- Time-restricted (has a clearly defined time frame or deadline)
- There are two types of goals:
- Short-term goals (can be completed immediately)
- Long-term goals (completed over a long period of time)
Implementation
- The actionable part of the process where the individual and medical team implement the care plan
- Indirect care refers to care that is given while away from the patient
Evaluation
- The last phase of the process where medical professionals assess and evaluate the success of the planning
Nursing Health Assessment
- Provides foundation for quality nursing care
- Requires skills such as:
- Cognitive skills (thinking process)
- Problem-solving skills (using scientific methods and experience)
- Psychomotor skills (doing)
- Affective/interpersonal skills (feeling, trust, and mutual respect)
- Ethical skills (being responsible and accountable)
Nursing Process
- A systematic, rational method of planning and providing nursing care
- Purpose:
- Identify a client's health care status
- Establish plans to meet the identified needs
- Deliver specific nursing interventions
- Ida Jean Orlando (1958) started the nursing process and defined it as a "systematic approach"
Types of Assessment
- Initial (performed within a specified time period, establishes a complete database)
- Problem-focused (ongoing process integrated with care, determines the status of a specific problem)
- Emergency (performed during physiologic or psychological crises, identifies life-threatening problems, identifies new or overlooked problems)
- Partial/ongoing (occurs after initial database is established, any problems initially detected in the client's body system are reassessed to determine any changes)
Assessment Activities
- Collecting data (past and present history, allergic reactions, past surgical procedures, folk healing practices, sleep patterns, religious practices, pain, nausea)
- Organizing data (format may be modified according to the client's physical status)
- Validating data (act of "double checking")
- Documenting data (records client's data)
- Goal (establish a database about the client's response to health concerns or illness)
