    Study Notes

    Blood Supply of the Hand

    • The radial and ulnar arteries are the primary blood supply to the hand.
    • The radial and ulnar arteries form two interconnected arches: the superficial palmar arch and the deep palmar arch.
    • The superficial palmar arch is mainly supplied by the ulnar artery, with contributions from the radial. It supplies the fingers through common palmar digital arteries, which then split into proper palmar digital arteries.
    • The deep palmar arch is primarily formed by the radial artery, with a contribution from the ulnar artery. It supplies the palmar metacarpal arteries and provides deeper blood supply to the hand muscles and bones.
    • The interconnected arches create an anastomotic network which ensures collateral blood flow to the hand and fingers even if one artery is compromised.
    • The medial hand (little finger side) is mainly supplied by the ulnar artery.
    • The lateral hand (thumb side) is primarily supplied by the radial artery, which branches into the princeps pollicis (thumb) and radialis indicis (index finger) arteries.
    • The radial artery is a major contributor to the deep palmer arch.

    Squamous Cell Carcinoma

    • Keratin pearls are a hallmark of squamous cell carcinoma.
    • UV exposure is a common risk factor for this condition.
    • Actinic damage often preceeds the development of this carcinoma.

    Erythema Multiforme

    • Target lesions are characteristic of erythema multiforme.
    • Herpes simplex virus (HSV) infection is a frequent trigger.
    • Drug reactions can also cause erythema multiforme.

    Plasma Cell Dyscrasia

    • Dense infiltrate of plasma cells on histological examination points to a plasma cell disorder.
    • Multiple myeloma is a common example of a plasma cell dyscrasia.
    • Other plasma cell abnormalities can also display significant plasma cell accumulation.

    Acantholysis

    • Loss of attachment between keratinocytes is termed acantholysis.
    • Pemphigus vulgaris is a condition associated with acantholysis.
    • Autoimmune response against desmosomes underlies acantholysis in pemphigus vulgaris.

    Type 4 Hypersensitivity

    • Eczematous dermatitis is mediated by type 4 hypersensitivity.
    • Spongiosis refers to intercellular edema in the epidermis.
    • Allergic contact dermatitis is a common cause of eczematous dermatitis.
