Quality control for histology forms (AP-HIS.A300) will be performed.

What must a technician do before distributing slides to a pathologist?

Microscopically check the slides

Specimens for special stains will be obtained from [patients].

True or false: The technician must select the appropriate control to be stained with the special stain ordered by the Pathologist/Resident?

True

What forms are used in quality control for special stains?

Quality control for special stains is performed using the Histology Quality Assurance Tracker (Appendix 1) and microscope maintenance quality control logs (Appendix 4, Appendix 8, and Appendix 10).

Microscope slides, appropriate reagents, and containers for the reagents will be used.

True or false: Patient results are only reported to the Pathologist/Resident?

False

What is the new reagent lot confirmation of acceptability?

The new reagent lot confirmation of acceptability is a form that is used to confirm that the reagent meets specification and positive staining criteria (Appendix 6).

When must special stains be repeated?

If a stain has to be repeated

What must the technician do when a new reagent lot is used?

Perform the corresponding stain on both the old and new reagent lot

True or false: The limit of procedure/AVIII. REFERENCE is current as of 22-Feb-2023 14:47?

True

The method for quality control of special stains has been [approved] and is currently in use.

What is the purpose of the Special Stains QC Log?

The purpose of the Special Stains QC Log is to document the case number, type of stain performed, and requesting pathologist for each stain performed.

All stains performed are to be annotated, indicating the case number, type of stain performed and requesting [pathologist] on the Special Stains Quality Control Log.

What is the current status of the method for quality control of special stains?

The method for quality control of special stains has been approved and is currently in use.

True or false: The reagent has been found to meet specification criteria and positive staining criteria?

True

What is the status of the AP-HIS.402 Quality Control of Special Stains (HIS)?

  • Specimens for special stains will be obtained from patients and controls will be used to ensure the accuracy of the staining results.
  • Quality control for histology forms (AP-HIS.A300) will be performed.
  • Microscope slides, appropriate reagents, and containers for the reagents will be used.
  • Quality control for special stains will be performed using the Histology Quality Assurance Tracker (Example) (Appendix 1).
  • Microscope maintenance quality control logs will be performed (Appendix 4, Appendix 8, and Appendix 10).
  • Special stains will have a new reagent lot confirmation of acceptability (Appendix 6).
  • Waterbath maintenance forms will be used (Appendix 7 and Appendix 8).
  • The technician performing special stains must print out a Special Stains QC Log.
  • The technician must select the appropriate control to be stained with the special stain ordered by the Pathologist/Resident.
  • All stains performed are to be annotated, indicating the case number, type of stain performed and requesting pathologist on the Special Stains Quality Control Log.
  • If a stain has to be repeated, note why the stain has to be repeated on the Special Stains QC Log as well as a separate entry when stain is repeated with new control results.
  • All slides will be microscopically checked by the technician before distributing to the pathologist.
  • Stains will be repeated at the very first opportunity and returned to the respective pathologist.
  • Patient results are to be reported to the Pathologist/Resident and the Special Stains Technician.
  • The technician will perform the corresponding stain on both the old reagent lot and the new reagent lot to confirm acceptability.
  • The confirmation is annotated on the Special Stains New Reagent Lot Confirmation of Acceptability form and reviewed by the Technical Supervisor.
  • The limit of procedure/AVIII. REFERENCE is current as of 22-Feb-2023 14:47.
