  • The reagent strip method for hemoglobinuria is often negative; however, hemosiderin can be found as yellow-brown granules in casts (Fig. 28-1).
  • Hemosiderin also appears in the urine sediment in diseases with a true side rosis of kidney parenchyma (hemachromatosis).
  • Although the existence of hemosideruria indicates a chronic hemolytic state, its presence is rarely needed to make the diagnosis of hemolysis; other tests, such as serum bilirubin, lactate dehydrogenase, and haptoglobin levels, will usually point to the correct diagnosis.
