Attendance Control Policy
- The Fort McDowell Yavapai Nation's attendance control policy aims to ensure that each department is fully staffed to meet its commitments.
Employee Responsibilities
- Employees are expected to control personal absences, including tardiness.
- Employees must personally notify their supervisor of any absence or tardiness within 30 minutes of the start of their scheduled workday.
- Employees must contact their supervisor each day for consecutive days of absence.
- Employees must notify the Human Resources Department if their supervisor is unavailable.
Requesting Time Off
- Employees must notify their department head a minimum of 3 working days in advance for personal time off, personal holiday, or leave of absence.
- Employees must follow the procedure above for bona fide emergencies.
Consequences of Absence
- Employees who are absent for 3 or more consecutive work days without prior authorization or timely notification will be considered as having voluntarily terminated.
- Employees absent for 3 or more consecutive work days due to a non-work-related illness or injury must provide an acceptable physician release to work without restriction.
Department Head Responsibilities
- Department heads are responsible for the timely recording of all employee absences and reviewing employee attendance records for corrective action.
- Department heads must stress the importance of regular attendance to employees and the effect of absence on fellow workers.
- Department heads must encourage dependability by recognizing employees with good attendance records and employees who improve their records.
Attendance/Absenteeism/Tardiness
- Absences counted as incidents include all time lost from the job, except for holidays, previously approved personal time off, and other approved leaves.
- An employee is considered tardy if not clocked in and at the work station at the start of the scheduled workday.
- Excessive absenteeism is defined as 3 incidents within a 30-day period, 5 incidents within a 90-day period, or 12 incidents within a 12-month period.
Disciplinary Procedure
- The disciplinary procedure for excessive absenteeism/tardiness follows a progressive sequence of written warnings, disciplinary suspension, and discharge/termination.
- Prior written warnings for attendance violations will be removed from the employee's file after 12 consecutive months without disciplinary action.
Recognition/Awards
- Employees with perfect attendance in a full calendar year will be recognized and receive a plaque, gift certificate, and/or similar award.
- Department heads will review employee records with the Human Resources Department to identify candidates for recognition.
