    Arterial Supply of the Brain

    • Brain blood supply comes from two systems: Vertebro-basilar system and Internal carotid system.

    Vertebro-basilar System

    • Arises from the 1st part of the subclavian artery at the root of the neck.
    • Ascends through the foramina transversaria of cervical vertebrae, exiting at the atlas vertebra.
    • Enters the foramen magnum and runs medially along the medulla oblongata.
    • Forms the basilar artery at the lower border of the pons.
    • Branches:
      • Anterior spinal artery: descends along the spinal cord's anterior median fissure, supplying the anterior 2/3 of its cross-sectional area, as well as the medulla oblongata's medial part.
      • Posterior spinal artery: descends along the spinal nerve's posterior roots, supplying the posterior 1/3 of the spinal cord's cross-sectional area.
      • Posterior inferior cerebellar artery (PICA):
        • Largest branch of the vertebral artery.
        • Travels backward below the medulla's olive, reaching the cerebellum's inferior surface.
        • Supplies:
          • The posterior part of the cerebellum's inferior surface, including the inferior vermis.
          • The inferior cerebellar peduncle.
          • The 4th ventricle's choroid plexus.
          • Medulla oblongata's lateral part.
          • Occlusion leads to lateral medullary syndrome ("PICA syndrome" or "Wallenberg's syndrome").
          • The posterior spinal artery may originate from it.
      • Medullary branches: Throughout its course.
