Tear Film
- The outermost lipid layer prevents tear evaporation, lubricates lid movement, contributes to optical properties, and maintains a tear-strip at the lid margin, increasing surface tension.
Middle Aqueous Layer
- This layer supplies oxygen to the avascular eye.
- It provides an antibacterial environment.
- It maintains a smooth surface and washes away debris toward the puncta.
Innermost Mucin Layer
- The mucin layer interacts with the lipid layer to stabilize the tear film.
- It helps collect loose cells, foreign particles, and bacteria in the bulbar conjunctiva.
Conjunctiva
- A thin, transparent mucous membrane lines the posterior surfaces of the eyelids and covers the anterior surface of the eyeball, excluding the cornea.
- It secretes mucus for eye lubrication.
- The conjunctiva has two parts: bulbar and palpebral.
- The bulbar conjunctiva covers the sclera, not the cornea.
- The palpebral conjunctiva covers the inside of the eyelids.
- The forniceal conjunctiva is a transition region between the bulbar and palpebral conjunctiva.
Eye Muscles
- Six muscles, four straight and two oblique, attach to the outer surface of the eye.
- The eye muscles facilitate movement in:
- Horizontal plane (toward or away from the nose)
- Vertical plane (elevation and depression)
- Torsional movement (tilting the top of the eye toward or away from the nose)
- The medial rectus is for adduction; the lateral rectus is for abduction.
- In the primary position, both rectus and oblique muscles contribute to vertical movement.
- During abduction, rectus muscles are the prime movers
- During adduction, oblique muscles are the prime vertical movers.
Eye Movement Dysfunction
- Strabismus is a turned eye (one eye turned towards the nose is called esotropia; one eye turned outwards is called exotropia).
- Other kinds of strabismus exist (hypotropia – downward eye movement, hypertropia- upward eye movement)
