Study Notes
Gluteal Region
- The gluteal region is located posteriorly to the pelvic girdle, at the proximal end of the femur.
- The muscles in this region move the lower limb at the hip joint.
Arterial Supply and Venous Drainage
- The arterial supply to the gluteal muscles is mostly via the superior and inferior gluteal arteries, which are branches of the internal iliac artery.
- Venous drainage follows the arterial supply.
Superficial Muscles
- The superficial muscles in the gluteal region consist of the three glutei (maximus, medius, and minimus) and the tensor fascia lata.
- These muscles mainly act to abduct and extend the lower limb at the hip joint.
- Gluteus Maximus is the largest and most superficial gluteal muscle, producing the shape of the buttocks.
- Gluteus Medius is a fan-shaped muscle that lies between the gluteus maximus and minimus.
- Gluteus Minimus is the deepest and smallest of the superficial gluteal muscles.
- Tensor Fascia Lata is a small superficial muscle that tightens the fascia lata, and abducts and medially rotates the lower limb.
Clinical Relevance: Gluteal Muscle Disorders
- Damage to the Superior Gluteal Nerve can cause paralysis of the gluteus medius and minimus, leading to pelvic instability.
- The Trendelenburg sign is a characteristic finding of gluteal muscle weakness, where the pelvis drops on the unsupported leg when standing on one leg.
Deep Muscles
- The deep gluteal muscles are a set of smaller muscles located underneath the gluteus minimus.
- These muscles laterally rotate the lower limb and stabilize the hip joint by 'pulling' the femoral head into the acetabulum of the pelvis.
- Piriformis is a key landmark in the gluteal region, dividing the region into an inferior and superior part.
- Obturator Internus forms the lateral walls of the pelvic cavity.
- The Gemelli muscles are two narrow and triangular muscles separated by the obturator internus tendon.
- Quadratus Femoris is a flat, square-shaped muscle, located below the gemelli and obturator internus.
Clinical Relevance: Landmark of the Gluteal Region
- The piriformis is an important anatomical landmark in the gluteal region, dividing the region into an inferior and superior part.
- The piriformis determines the name of the vessels and nerves that supply the area.
