Adult Trauma Alert Criteria
- Criteria apply to individuals aged 16 years and older.
- Trauma Alert patients are transported to the nearest appropriate Trauma Center.
Red Criteria (any one criteria met requires transport)
- Active airway assistance required beyond supplemental oxygen.
- Best Motor Response (BMR) ≤ 4, Glasgow Coma Score (GCS) < 12, paralysis, or loss of sensation.
- Absence of radial pulse with sustained heart rate > 120 bpm or systolic blood pressure < 90 mmHg.
- Two or more long bone fractures (e.g., humerus, femur, etc.).
- 2nd or 3rd degree burns covering > 15% of total body surface area (TBSA), or amputations, or penetrating injuries to vital areas.
- Patients on anticoagulants experiencing head trauma with GCS < 15.
Blue Criteria (two or more criteria met require transport)
- Patients aged > 55 years.
- Respiratory rate > 30 breaths per minute.
- BMR ≤ 5.
- Sustained heart rate > 120 bpm.
- Long bone fractures from motor vehicle collisions (MVC) or falls > 10 feet.
- Major degloving injuries, avulsions > 5 inches, or gunshot wounds to extremities.
- Ejection from motor vehicle or related severe impacts.
Pediatric Trauma Alert Criteria
- Criteria apply to individuals under 16 years of age.
- Trauma Alert patients are transported to the nearest appropriate Trauma Center.
Red Criteria (any one criteria met requires transport)
- Assisted or intubated trauma patients.
- Altered mental status.
- Paralysis or suspected spinal injuries.
- Non-palpable radial or femoral pulse and/or systolic blood pressure < 50 mmHg.
- Open long bone fractures, multiple fractures, or pelvic fractures.
- Major tissue disruption or amputations.
- 2nd or 3rd degree burns covering > 10% TBSA.
- Penetrating injuries to the head, neck, or torso.
Blue Criteria (two or more criteria met require transport)
- Patients weighing < 11 kg (24 lbs) with distress indicated by weight-based tape.
- Amnesia or known loss of consciousness.
- Palpable carotid or femoral pulses with absent radial pulse, indicating systolic blood pressure between 50-90 mmHg.
- A single, closed long bone fracture.
- Ejection from a motor vehicle or death of an occupant in the same vehicle.
Paramedic Judgment
- Paramedic discretion may override established criteria for transport.
- Situations involving prolonged extrication, penetrating wounds without distal pulses, or unknown exit paths on gunshot wounds qualify for Trauma Center transport.
- Once declared, a Trauma Alert cannot be downgraded by any personnel.
