All of the caries, debris, and necrotic material
What is the function of the endodontic spoon excavator?
To excavate the base of the pulp chamber
How many roots are typically present in premolars?
Two roots with three canals
What can occur if calcified or metallic debris is left in the pulp chamber?
The canal may be obstructed
What is the purpose of the 'drop in' feeling during access cavity preparation?
To locate the canal orifices
What is the direction of movement of the round bur during access cavity preparation?
Buccolingual direction
What is the shape of the access cavity for a maxillary central incisor?
Rounded triangular with base facing the incisal aspect
What is the main difference between the access cavity of a maxillary lateral incisor and a maxillary central incisor?
Size of the access cavity
What is the shape of the access cavity for a maxillary canine?
Oval shape with greater diameter labiopalatally
What is the main difference between the access cavity of a mandibular incisor and a maxillary incisor?
Shape of the access cavity
What is the shape of the access cavity for a mandibular canine?
Oval shape with greater diameter labiopalatally
What is the error caused by failure to notice the lingual axial inclination of anterior teeth?
Gouging of the labial wall
What is the error caused by failure to complete the convenience extension toward the incisal prior to the entrance of the shaft of the bur?
Labiocervical perforation
What is the error caused by failure to remove pulp debris due to incisally under-extended cavity?
Discoloration of the crown
Study Notes
Maxillary Central Incisor
- Access cavity is rounded triangular in shape with base facing the incisal aspect
- Width of the cavity depends on the distance between mesial and distal pulp horns
- Shape may change to oval in mature teeth due to less prominent pulp horns
Maxillary Lateral Incisor
- Shape of access cavity is similar to maxillary central incisor but smaller in size
- Rounded triangular or oval shape
Maxillary Canine
- Access cavity shape is oval with greater diameter labiopalatally
- No pulp horns present in canines
Mandibular Incisors
- Access cavity is smaller in size compared to maxillary incisors
- Shape is long oval with greater diameter incisogingivally
- Different from maxillary incisors in size and shape
Mandibular Canine
- Access cavity is similar to maxillary canine but smaller in size
- Root canal outline is narrower in mesiodistal dimension
- Two canals are often present in mandibular canine
Errors during Endodontic Cavity Preparation
- Gouging the labial wall due to failure to notice lingual axial inclination
- Gouging the distal wall due to failure to notice mesial axial inclination
- Labiocervical perforation due to incomplete convenience extension
- Discoloration of the crown due to failure to remove pulp debris
- Errors in intraradicular cavity preparation due to incomplete authority over enlarging instruments
Access Cavity Preparation for Premolars
- Pulp chamber is located between buccal and lingual cusp tips
- Two pulp horns are located within the peaks of their cusps
- Penetrate enamel with No. 4 round bur directed parallel to long axis of tooth
- Remove roof of pulp chamber using a round, tapered fissure or non-end cutting bur
Toilet of the Cavity
- Remove all caries, debris, and necrotic material from the pulp chamber
- Debris left in the chamber can obstruct the canal and increase bacterial population
Instruments for Access Cavity Preparation
- Endodontic Explorer: helps locate canal orifices and determine canal angulation
- DG 16: designed to identify canal orifices and determine canal angulation
- Endodontic Spoon Excavator: used to excavate the base of the pulp chamber
This quiz covers the anatomy of access cavities for maxillary central incisors and lateral incisors, including their shape, size, and relationship to pulp horns.
