Is presented together with an original copy of the contract within 3month maximum from signing the contract ( Why ? ) Labor Law no. 12 for 2003 AUC Nermeen Roushdy 11 Labor Law – SI Forms SI Form # 2 During the month of January of each year, the employer is committed to submit to the labor legal authorities , data including the following; i. Number of workers ii. Their educational degrees iii. Positions ( Job Title ) iv. Ages v. Nationalities vi. Fixed Pay vii. Variable pay The form also includes the following : i. Current vacant positions, ii. All changes or amendments that occurred , iii. Estimated vacancies during the year The form presented to the labor office classified by education.. Is signed by Employer is case of : End of Contract, Retirement, Death of Employee, some disability cases, Lay Off , Downsizing & Dismissal iii. How is Medical check up is processed Labor Law no. 12 for 2003 AUC Nermeen Roushdy 14 Form 2 During the month of January of each year, the employer is committed to submit to Labor ;the labor legal authorities , data including the following Registration number of workers Form their educational degrees positions ages nationalities wages The form will also include vacant positions, all changes or amendments that might has occurred in addition to expected vacancies during the year ( مادة ) 15 يلتزم صاحب العمل فى المنشآت القائمة وقت تطبيق أحكام هذا القانون ،وتلك التى تنشأ مستقبال بأن يرسل إلى الجهة اإلدارية المختصة التى يقع فى دائرتها محل العمل خالل خمسة عشر يوما من تاريخ العمل بهذا القانون أو من تاريخ بدء العمل بالمنشأة -على حسب األحوال -بيانا مفصال بعدد العمال طبقا لمؤهالتهم ومهنهم وفئات أعمارهم وجنسياتهم ونوعهم واألحوال التى يتقاضونها .وعليه خالل ثالثين يوما من تاريخ شغل الوظيفة التى خلت لديه أن يعيد إلى الجهة اإلدارية شهادة قيد العامل الصادرة منها بعد استيفاء البيانات المدونة بها ،وعليه تدوين رقم شهادة القيد وتاريخها أمام إسم العامل فى سجل قيد العمال بالمنشأة . وعلى صاحب العمل فى المنشآت المشار إليها فى الفقرة األولى يرسل إلى ذات الجهة خالل شهر يناير من كل عام البيانات اآلتية : ما طرأ من تعديالت على البيانات الواردة فى الفقرة السابقة . .1 عدد الوظائف الشاغرة بسبب اإلحالل و التوسعات الجديدة . .2 بيان بتقدير االحتياجات المتوقعة موزعة بحسب الحالة التعليمية و المهنية خالل العام التالى . .3 Labor Law no. 12 for 2003 AUC Nermeen Roushdy 15
