HOFFMAN ESTATES FIRE DEPARTMENT STANDARD OPERATING GUIDELINES SOG Category & Identification Number: EFFECTIVE DATE: ADMINISTRATIVE – 024 January 1, 2022 NIMS COMPLIANT SOG Title: Emergency Incident Reporting Format Revision: 5 APPROVED BY: NUMBER OF PAGES: Re-evaluation Date: Alan Wax January 1, 2024 Fire Chief 5 PURPOSE POLICY This Administrative Guideline is intended to standardize the reporting of emergency incidents. All reports required by this policy shall be completed and submitted to the Shift Commander by the end of the duty day unless arrangements are made with the Shift Commander for a later submittal. All reports should include the following basic information as well as any additional information which would clearly illustrate the occurrences pertaining to the incident.  The units that responded to the incident.  What the first arriving unit(s) found on arrival.  Who was Command at the incident?  The actions of all companies that participated in the emergency activities at the scene.  Any notifications made as a result of the incident.  The final disposition of the incident.  The cause and origin of the fire incident (if known). PROCEDURE An NFIRS report shall be completed immediately following each emergency incident. Except as directed otherwise by the Shift Commander, the duty to complete an incident NFIRS report shall be as follows: For fire incidents, the first arriving engine officer shall complete, or shall direct the completion of a NFIRS report for the incident. For emergency medical calls when an ambulance responds by itself to the incident, its crew shall complete an NFIRS report for the incident. 1 For emergency medical incidents involving other apparatus responding to or operating at the emergency scene, the still district engine officer shall complete, or shall direct the completion of a NFIRS report for the incident. Due to the response guidelines of the Fire Department, this may result in the completion of the NFIRS report by the truck officer, squad company member, or ambulance crew. The narrative of the NFIRS report shall include the actions of all companies working at or which responded to the emergency incident and not just the actions of the unit which is reporting the incident. For all other incidents, the company officer of the still district engine shall complete, or shall direct the completion of, a NFIRS report for the incident. Example: “E21, E22, T22, S22, A21 and B6 responded to a reported structure fire at 250 Flagstaff Lane. Upon arrival of E21 and A21 light smoke was found coming from a one-story residence. E21 assumed command. All other companies were ordered to stage. E21, upon entry into the structure, found a small fire in the kitchen had been extinguished prior to our arrival by the occupants of the home. E21 removed the cabinets around the stove and placed them in the back yard. T22 was requested to ventilate the house. All other companies were returned to quarters. The fire was caused by unattended cooking materials. Upon completing the ventilation of the structure, all companies returned to quarters in service. The home was turned over to the owners and Code Enforcement was notified.” SUPPLEMENTAL REPORTS Companies which perform services at the scene of structure fires, or other incidents when directed by the Shift Commander, shall complete a supplemental narrative describing their actions unless directed otherwise by the Shift Commander. A Fire/Narrative Report shall be utilized and the completed report using the NFIRS Reporting Software. Example: “T22 was assigned by command to extend the main ladder to the roof of 26 Golf Rose Center to ventilate the roof over the fire. Company personnel used a chain saw to cut a 4’ x 8’ hole for ventilation. Upon completion of this assignment, we were assigned to shut off utilities and provide salvage work inside the building.” The Incident Commander shall, upon completion of the narrative by the first arriving officer and the review of the supplemental reports from other companies, complete an overall narrative of the events that transpired at the emergency incident including, but not limited to, the actions of all companies working at the incident, benchmark times, the location and size of all lines laid, the location of all apparatus utilized at the incident (unless shown on the drawing of the incident scene) including the command post, all notifications requested, ventilation methods utilized, the estimated dollar loss of the incident and the cause and origin of the fire if known. This report shall be submitted on a Fire/Narrative Report in the NFIRS Reporting Software. The tactical worksheet completed for the incident shall be attached to the Incident Commander’s overall report. 2 Example: “District 21 and 22 Companies and Bat 6 responded to a reported structure fire at 1800 Oakmont Road. E22 arrived first on the scene and reported a fire on the second floor of the structure. E22 assumed command and led out with a 1-¾” preconnect making their own hydrant connection. T22 was assigned ventilation duties and S22 was assigned primary search. Bat 6 arrived on the scene and assumed Command. E21 and A21, upon their arrival, were assigned to bring a second preconnected hose line in as a backup line. A Code 4 was requested by Command and E24, A23, A24 reported to the scene. All units staged on arrival. 601 responded on the Code 4 and was assigned as the Systems Officer (610). 602 responded to the scene and was the Operations Chief. S22 reported an “All Clear” on the primary search at 1700 (10 minutes after their arrival) and an “All Clear” on the secondary search at 1722. The fire was “Under Control” at 1715 hrs. The fire was confined to a second floor bedroom, but there was smoke damage through- out the second floor and water damage on both floors of the two story frame residence. The cause of the fire was a child playing with matches as determined by Inspector Neil who was requested to the scene. The total loss is estimated at $35,000. Code Enforcement and Utilities were requested to the scene.” FIRE CASUALTY REPORT An NFIRS Casualty Report shall be completed on each individual injury or death associated with a fire emergency incident. The person completing the NFIRS Report will be prompted to complete a Casualty Report whenever an injury or death is noted in the appropriate sections of the main NFIRS report. It should be noted that an incident number should also be generated and a NFIRS report should be completed for all injuries, including those that are not associated with an emergency incident. HAZARDOUS MATERIALS REPORT An NFIRS Hazardous Materials Report shall be completed when prompted by the main NFIRS report. The need for this report is determined by the information supplied in the “Type of Situation Found”, section of the main NFIRS report. PICTURES The inclusion of pictures with a fire incident NFIRS report aids in documenting the severity or unusualness of the incident. The Incident Commander shall be responsible to take or assign the taking of pictures of the fire building or area at any of the following incidents: 3  Any structure fire requiring the use of a preconnected or larger hose line(s) for fire extinguishment.  Any structure fire involving death or injury.  Any outside fire of unusual nature or involving death or injury.  Any vehicle fire involving death or injury. Pictures shall be taken utilizing department photographic equipment (including iPhones/iPads) which depict the extent of fire damage sustained by the exterior and interior of the fire building, vehicle or area. Any pictures shall be digitally attached using the Records Management System (RMS). A brief description of each picture shall be included. Additional pictures may be taken by the fire investigator, but these pictures do not replace the pictures, which the Incident Commander is responsible to secure. DRAWINGS For all structure fires requiring the use of a preconnected or larger hose line(s), a site plan, floor plan or combination drawing shall be completed. The drawing(s) shall depict the following:  The location of all Fire Department equipment actively involved in the incident.  The location of the command post/vehicle.  The basic building outline.  The basic floor plan of the portion of the structure affected by the fire.  The location of any fire fatalities found inside/outside of the structure.  The location and sizes of all hose lines (attack, relay or supply) laid during the incident.  The street or streets that the structure is located on.  A reference to “north”.  A notation that the drawing is “Not to Scale”. All drawings will be scanned and attached to the NFIRS report in the RMS. In addition, the electronic copies will be stored on the Village’s Computer Network designated for Records Storage of Fire Incidents. CONTINUOUS QUALITY IMPROVEMENT Report accuracy is of paramount importance and, therefore, shall be subject to a continuous quality improvement (CQI) process. After a NFIRS report has been completed and marked “Crew Finished”, the Shift Captain will review the reports that have been completed during their shift. Depending on the accuracy of the report, the Shift Captain will mark the report as “Crew to Review/Edit” so the crew can make necessary changes or “QI Complete/Complete”. 4 EXCEPTIONS REFERENCES DEFINITIONS End of Document 5
