ORGANIZATIONAL COMMUNICATION ORGANIZATIONAL COMMUNICATION  is the exchange of information, views, and ideas within and outside of an organization.  Communication moves in three directions: upward, downward, or horizontally.  The purpose of organizational communication is essential to the inner workings of a business. It's a subfield of all communication studies. Organizational communication helps with: 1. Completing tasks with the maintenance of policy 2. Developing relationships where messages are directed relating to the company culture 3. Putting changes into place through individual and organizational creativity 4. Accomplishing tasks relating to sales, services, and production 5. Controlling operations of the organization BUSINESS WRITING is a type of writing that is used in a professional setting. It is a purposeful piece of writing that conveys relevant information to the reader in a clear, concise, and effective manner. It includes client proposals, reports, memos, emails, and notices. Proficiency in business writing is a critical aspect of effective communication in the workplace. TYPES OF BUSINESS WRITING 1. Instructional  The instructional business writing type is directional and aims to guide the reader through the steps of completing a task. A user manual falls aptly under the instructional category, as well as a memo issued to all employees outlining the method of completing a certain task in the future. 2. Informational  Informational business writing pertains to recording business information accurately and consistently. It comprises documents essential to the core functions of the business for tracking growth, outlining plans, and complying with legal obligations.  For example, the financial statements of a company, minutes of the meeting, and perhaps the most important, report writing. 3. Persuasive  The goal of persuasive writing is to impress the reader and influence their decision. It conveys relevant information to convince them that a specific product, service, company, or relationship offers the best value.  Such a type of writing is generally associated with marketing and sales. It includes proposals, bulk sales emails, and press releases. 4. Transactional  Day-to-day communication at the workplace falls under the transactional business writing category. The bulk of such communication is by email, but also includes official letters, forms, and invoices. TYPES OF REPORT IN NURSING CHANGE-OF-SHIFT REPORTS (CSR) This type of reporting most commonly using.  The report provides continuity of nursing care among nurses who are caring for a client. TRANSFER REPORTS  Patient’s are often Transfer from one unit to another to receive different levels of care and treatment. Incident reports  An incident report is a formal documentation of any unexpected event or error that occurs during patient care. This report is a key component of risk management and is used to analyze the causes of the incident to prevent future occurrences. Telephone reports telephone reports involve conveying important patient information over the phone, often when immediate attention is required or during off-hours. These reports are essential for ensuring that key patient data is communicated in a timely manner. HEALTH INFORMATION SYSTEM Health Information System Health Information has been defined as information about people’s health and how the community, government and non- governmental organizations use the information to plan and provide health care and services. Health information describes the incidence, prevalence and causes of major diseases. It also tells us about the availability and the effectiveness of curative services. 3. It has also been defined as a system that collects and presents health information and is responsible for creation and maintenance of a data base 4. Another definition of health information is that it refers to organized and interpreted data about health status, resources, activities and events as related to health SENTINEL EVENTS  Any unanticipated event in a healthcare setting resulting in death or serious physical or psychological injury to a patient or patients, not related to the natural course of the patient's illness  An unexpected incident, related to system or process deficiencies, which leads to death or major and enduring loss of function for a recipient of healthcare services  A sentinel event is a Patient Safety Event that reaches a patient and results in any of the following: Death, Permanent harm, Severe temporary harm and intervention required to sustain life TYPES OF SENTINEL EVENTS 1. Surgical Events ( wrong body part/ patient/ procedure, retained instrument, death during the procedure, anesthesia related events) 2. Device or Product events (contaminated drugs and device, unintended use, breakdown or failure) 3. Patient protection events (infant discharge to wrong person , elopement, suicide, attempted suicide, self-harm, intentional injury, nosocomial infection, medical gas ) 4. Environmental events ( burn, slip, trip, fall, electric shock, use of restrains and bed rails) ANECDOTAL RECORD is an observation that is written like a short story. They are descriptionsof incidents or events that are important to the person observing. Anecdotal records are short, objective and as accurate as possible are a record of some significant item of conduct, a record of an episode in the life of students, a word picture of the student in action, a word snapshot at the moment of the incident, any narration of events in which may be significant about his personality. INCIDENT REPORT  an incident report or accident report is a form that is filled out in order to record details of an unusual event that occurs at the facility, such as an injury to a patient.  The purpose of the incident report is to document the exact details of the occurrence while they are fresh in the minds of those who witnessed the event.  This information may be useful in the future when dealing with liability issues stemming from the incident  The incident doesn’t have to have caused harm to a patient, employee, or visitor, but it’s classified as an “incident” because it threatens patient safety. KARDEX  is for a filing system for nursing records and orders that was held centrally on the ward and contained all the nursing details and observations of patients that had been acquired during their stay in hospital. PATIENT CHART/RECORD  It is a permanent record of the patient’s information.  Tracks the progress of the patient’s condition during the hospitalization as well as the status upon discharge. It serves as an information sheet of the medications and procedures rendered to the patient.  Legal evidence for cross-examination whenever complaints or malpractice claims have been sighted out.  It serves as the evidence of continuity of care.  It serves as a research material for the retrospective study. 201 FILE  is basically the employees’ profile in the organization’s records.  tracks a wide range of personnel information and provides sophisticated reports and its richly-presented variants from corporate, groups and down to the individual worker.  it includes an employee’s personal details like full name, address, date of birth, and others.  some other details that a “201” file can hold are: the employees’ SSS number, TIN , PhilHealth registration, HDMF number, educational transcripts/diplomas, performance appraisals, issued corrective actions, clearances and many others.
