SPECIALIST TRAINING ARTICLE 12-20 Preparation of a thesis is mandatory to take the specialization training graduation exam. To complete the specialization training: a) The thesis must be accepted, b) The specialization training period and rotations related to the specialization branch specified in the schedule must be completed, c) The section of the specialization training report card that determines the core training curriculum must be approved by the relevant academic commission, ç) Success in the specialization training completion exam is required. DECISION OF THE PHARMACY SPECIALIZATION BOARD DATED 15/06/2022 https://dosyamerkez.saglik.gov.tr/Eklenti/44827/0/euk-karar-105--klinik-eczacilik- uzmaninin-gorev-ve-yetkileripdf.pdf DUTIES AND AUTHORITIES OF THE CLINICAL PHARMACY SPECIALIST To be able to access patient records during the treatment process within the framework of the legislation on the protection of personal data for drug review and drug treatment monitoring, to obtain the patient's drug and disease history, and to communicate verbally/written with the patient/patient relatives within this scope, To contribute to rational drug use, To contribute to the management of drug-related problems (drug-drug and drug-food interactions, drug adverse effects, treatment compliance, drug use without indication, drug duplication, drug not being used for an existing indication, insufficient or excessive drug use, use of a drug that is not suitable for the patient) and drug errors, DUTIES AND AUTHORITIES OF THE CLINICAL PHARMACY SPECIALIST To evaluate patients’ medications in a structured manner (“medication review”) and monitor drug therapy for effectiveness and safety (“medication monitoring”) in order to optimize the effect of medications, minimize the number of drug-related problems and improve the cost- effective use of medications, To contribute to the monitoring and interpretation of blood drug concentrations (“therapeutic drug monitoring”) in order to optimize treatment, especially with drugs with a narrow therapeutic range, To provide drug counseling services to healthcare personnel and patients/patient relatives, To provide patient education services in order to prevent drug-related problems, improve treatment compliance and treatment outcomes, DUTIES AND AUTHORITIES OF THE CLINICAL PHARMACY SPECIALIST To record the services provided, To take part in committees such as drug management team, rational drug team, nutrition support team, formulary committee, antibiotic/infection committee in the institution where he/she works, with the approval of the hospital management, In case of detection of adverse drug reactions, drug errors or drug ineffectiveness, to report these to the relevant units with a “clinical pharmacist consultation note”, To support the implementation of clinical pharmacy practice/internships of pharmacy faculty students and the education of clinical pharmacy specialization students in the institution where he/she works, when necessary, DUTIES AND AUTHORITIES OF THE CLINICAL PHARMACY SPECIALIST Participating in rounds with the healthcare team, Informing the views and suggestions regarding treatment via “clinical pharmacist consultation note” or in written form as a consultation upon the request of the attending physician, Determining the problems related to medicines and sharing the suggestions for their solution with the attending physician and other healthcare professionals when necessary, Planning and/or conducting research/projects in clinical pharmacy practices, Organizing training seminars related to the field for hospital staff and students.
