URC Doctor Note Go to GoHealth with the website you see in the picture below.  Search the clients name or ID number in the search box on GoHealth. Wait until the name pops up and then select the client's name.  It may be beneficial to you to bookmark the link for easy access later.  Make sure the drop down bar on the side, under the person's name,  says "CRS URC." Click on "Medical," found below the client search on the screenshot below.  Click on URC Doctor Note. URC Doctor note is at the bottom of third column in the screenshot below.  Here is what the URC Doctor note form looks like.  Fill out the form completely and then finalize at the bottom.
