REPUBLIC OF THE PHILIPPINES POLYTECHNIC UNIVERSITY OF THE PHILIPPINES OFFICE OF THE VICE PRESIDENT FOR ACADEMIC AFFAIRS COLLEGE OF SCIENCE LABORATORY – CEA BUILDING OFFICE ORDER NO. 01, Series of 2024 FOR/TO: ALL CONCERNED SUBJECT: POLICIES AND GUIDELINES ON THE USE OF COLLEGE OF SCIENCE LABORATORY – CEA BUILDING DATE: August 26, 2024 A. General Policies 1. The College of Science Laboratory – CEA facilitates experiments for the courses General Physics, General Chemistry, Chemistry for Engineers, and Physics for Engineers for the the College of Engineering and the Institute of Technology students. This is done under the direct supervision of the assigned faculty from the Department of Physical Sciences. Additionally, undergraduate research related to chemistry and physics is supported based on the needs of the researchers. 2. The College of Science Laboratory – CEA is open from 7:30 AM to 7:30 PM Monday through Friday, and from 7:30 AM to 4:30 PM on Saturdays. The schedule for the utilization of the laboratory rooms is determined by the Laboratory Head prior to the start of every term. Any use of the rooms beyond the scheduled times must be requested and approved in writing before use. 3. Students can only access the laboratory rooms when the faculty-in-charge is present in the area. 4. All students who will use the laboratory rooms, equipment, and/or chemicals are required to attend the Policies and Guidelines on the use of the College of Science Laboratory - CEA Building Orientation to be conducted by the faculty-in-charge during the orientation period as stated in the course syllabus. Moreover, the students are also required to get a passing score on the Policies and Guidelines on the us of the CS Laboratory – CEA Building examination before participating in any laboratory activity. 5. Use of the laboratory rooms are strictly implemented as scheduled. Unauthorized access to the laboratory room outside of scheduled hours is prohibited to ensure safety and maintain order. 6. For the use of laboratory facilities, equipment, glasswares, laboratory materials and chemicals, the students must first accomplish the Borrower’s Slip at least three (3) days before use. 7. The students should adhere to all safety protocols, policies and guidelines as outlined in the laboratory manual and the faculty in-charge. 8. All laboratory activities should be conducted in the presence of the assigned laboratory faculty. 9. Any experiment or use of laboratory equipment should not be started without the faculty supervision. B. Operational Guidelines 1. Before the conduct of a laboratory activity a. Completion of any required safety training should be confirmed. Attendance at pre- laboratory briefings or safety meetings conducted by the faculty-in-charge should be ensured. b. The lab manual and any specific instructions provided by the faculty should be read and understood. Experimental protocols and safety guidelines related to the experiment should be reviewed. A pre-laboratory report is required to be submitted before the conduct of an experiment. c. All necessary personal protective equipment (PPE) such as laboratory gows, safety goggles, and gloves should be available and in good condition. The CS Laboratroy – CEA maintains the “No appropriate PPE, No experiment” Policy. d. Emergency equipment (eyewash stations, safety showers, fire extinguishers) should be verified for functionality and accessibility. e. All required chemicals should be available, properly labeled, and reviewed with Safety Data Sheets (SDS). f. Laboratory equipment should be inspected for proper functioning. The work area should be clean and organized before starting. Any unnecessary items should be removed. h. Necessary equipment and materials should be arranged as per the experimental procedure. i. As a general rule, the following activities are not allowed inside the laboratory rooms: i. Eating, drinking, chewing gum and smoking ii. Horseplay of any sort iii. Sleeping iv. Listening to music using any form of gadget v. Playing computer games vi. Charging of cellular phones and laptops vii. Transfer of a portable equipment without approval from the laboratory head viii. Wearing neckties, dangling clothing or jewelry. ix. Wearing of shorts and open shoes. 2. During the conduct of a laboratory acitvity a. Required PPE should be worn and properly fitted at all times. b. Ensure long hair is secured and avoid wearing loose clothing or jewelry that may interfere with lab activities. c. Experimental procedures and safety protocols outlined in the laboratory manual should be strictly adhered to. d. Chemicals should be handled with care, using appropriate tools and techniques to avoid spills or accidents. e. Chemicals should not be mixed unless instructed. Chemicals should not be used if their properties or hazards are not understood. f. Laboratory equipment should be used according to the manufacturer's instructions and experimental procedures. g. Equipment should be continuously monitored during experiments to detect any malfunctions or issues. h. Emergency procedures and the location of emergency equipment should be known. i. Any accidents, spills, or safety concerns should be immediately reported to the faculty, laboratory staff or laboratory head. j. Chemical wastes should be segregated according to type and placed in designated containers. k. Physical wastes, such as broken glass, should be disposed of in appropriate containers labeled for such waste. l. Specific disposal instructions provided by the faculty or laboratory administrator should be followed. 3. After the conduct of a laboratory activity a. All waste should be properly disposed of according to the laboratory’s procedures. b. Equipment should be cleaned and returned to its designated storage location. c. Work surfaces and equipment should be wiped down with appropriate cleaning agents. d. All personal belongings should be collected, and any items not belonging in the lab should be removed. e. Any lost or damaged items should be reported to the faculty. f. The laboratory notebook should be completed with all relevant data, observations, and conclusions. g. Completeness and accuracy of the lab notebook should be double-checked. h. Any accidents or near-misses should be documented in detail and reported to the faculty in-charge. i. Any feedback or notes provided by the faculty should be reviewed for improvements in future lab sessions. j. Any issues or safety concerns encountered during the laboratory activity should be assessed and suggestions for improvements should be made. k. Participation in discussions regarding updates to safety procedures or protocols should be ensured. l. Confirmation that all wastes has been properly disposed of and no hazardous materials remain in the laboratory should be ensured. m. Any recyclable materials should be sorted and processed according to the lab’s recycling protocols. n. Doors of the laboratory room should be closed and locked by the faculty-in-charge after use. o. All ceiling fans shouls be turned off before leaving the laboratory after use. p. If the class falls into 4:30PM to 7:30PM class schedule, make sure to plug off all exhausts fans and all windows are properly closed. Page 2 of 3 4. In case of breakages, damages and losses a. A student accomplishes three (3) copies of the the Laboratory Damage Form duly signed by all members of the group. b. The student requests the signature of the faculty-in-charge and returns back the properly accomplished form to the Laboratory Staff. c. The students are given one (1) week to replace the material with the same specification of the one that was broken, damaged or lost. 5. In case of accident, fire or chemical spill: a. The student must report immediately the accident, fire, chemical spill or other incident to the faculty-in-charge, laboratory staff or laboratory head. b. The student must accomplish the Incident Report Form. c. The student requests the signature of the faculty in-charge and returns back the properly accomplished form to the Laboratory Staff. d. The student should attend meetings and inquiries regarding the incident as needed. C. Safety and Emergency Procedures 1. In times of emergencies, the following basic procedures should be observed: a. Stay calm and assess the situation. Remain composed to ensure clear thinking and effective action. Quickly evaluate the nature and severity of the incident or emergency. b. Inform the faculty, laboratory head, and/or laboratory technician immediately. Provide a clear description of the incident and any immediate hazards. c. Adhere to established safety protocols specific to the type of incident (e.g., chemical spills, electrical issues, or fires). d. If the situation poses a serious risk (e.g., fire, hazardous chemical spill), evacuate the laboratory immediately. Follow the designated evacuation routes and assembly points. Do not use elevators during a fire evacuation. e. Provide first aid to injured individuals if you are trained and it is safe to do so. Avoid moving severely injured individuals unless necessary for safety. 2. Use safety equipment as needed. Fire extinguishers are available near the doors of the laboratory rooms, and each chemistry laboratory is equipped with a laboratory shower and eyewash station. 3. Call emergency services if the situation requires professional medical or fire assistance. Provide detailed information about the location and nature of the emergency. Additional policies may be introduced as situations arise, taking into account the best interests of the CS Laboratory. These policies are intended to guide us in maintaining a safe and efficient working environment. 2. A serious breach of any safety rules or laboratory policies may result in immediate removal from the laboratory. If a student repeatedly ignores the safety rules or laboratory policies on three or more occasions, they will be summoned for disciplinary action along with their guardian. An appropriate penalty may be imposed, and the student's grade may be significantly impacted at the discretion of their laboratory insructor or professor. EFFECTIVITY: Effective immediately upon the approval of the College Dean on or before the 1st Semester, 2024-2025 starts on September 9, 2024. Prepared by: Endorsed and Approved by: (Sgd) Engr.
