Patient Rights/ Responsibilities 1. PURPOSE: 1.1 To ensure that patient’s rights and responsibilities are observed to assist patients to make basic decisions about medical and health care services which they receive from physicians and /or other health care providers. 1.2 To ensure that patients / families and center Staff are aware of the rights and responsibilities of all patients. 2. DEFINITIONS: 1.1. Patient Rights – privileges / rights conferred to a patient during their stay in the center. 1.2. Responsibilities- accountabilities expected from a patient / family during their stay in the center. 3. POLICY 3.1 In order to protect the personal welfare and safeguard the dignity of patient, the Center have adopted the patient rights and responsibilities. 3.2 That all patients receive information and education about their rights and responsibilities, however, in case that patient’s condition prevent him / her to understand these rights, the responsible family member will be the one to receive these rights from the appropriate staff. 3.3 All new staff will be oriented to patient rights and responsibilities. 3.4 All patient / family education related to patient rights and responsibilities shall be documented by the member of the health team at the Multidisciplinary Patient /Family Education Form. 3.5 The Patient Rights and Responsibilities must be explained according to patient’s understanding and level of education (in Arabic or English), and for non-Arabic or non-English speaking patients, efforts must be made by the center for patient to understand these rights by utilizing staff with similar nationality and language to explain these rights to the patient. 4. PROCEDURES: 4.1. PATIENT RIGHTS: 4.1.1. Access to and transfer of care, continuity of care: 4.1.1.1. To receive reasonable available treatment and / or services which are medically indicated, within the capability and capacity of Center. 4.1.1.2. When medically permissible, patients may be transferred to another facility. 4.1.1.3. The right to change or transfer to other Center as per patient’s or family request. 4.1.1.4. The right to ask to leave the center, against the physician’s advice. 4.1.1.5. Prior to leaving the center, patients continuing health care requirements are explained to them by their physician /designee and other members of the health care team. 4.1.1.6. At the end of the visit the patient has the right to have medicine prescriptions, follow-up appointments and all the information and the training needed to be able to take care of self at home (if the case requires). 4.1.1.7. Physicians give patients accurate and honest information for: 4.1.1.8. their illness 4.1.1.9. the proposed treatment 4.1.1.10. potential benefits 4.1.1.11. potential complications 4.1.1.12. likelihood of success of treatment 4.1.1.13. Their attending physician’s name and other staff names involved in their care 4.1.1.14. To receive reasonable available treatment and / or services which are medically indicated, within the capability and capacity of Center. 4.1.2. Considerate & Respectful Care: The right to considerate care, with full respect of patient’s dignity, regardless nationality, color, age, sex, religion, and disability 4.1.3. Identity of Care Providers: The right to know by name the physicians, nurses, and staff members involved in the treatment 4.1.4. Involvement in Care: The right to participate in determining the care provided recommended treatment or procedures 4.1.5 Informed Consent: Patients receive from their physician information necessary to give informed consent prior to the start of any procedure. 4.1.6 Refusal of Treatment: A competent adult patient or the patient’s properly designated agent has the right to refuse treatment to the extent permitted by law and to be informed of the medical consequence of such refusal. 4.1.7 Privacy and Confidentiality: 4.1.7.1 Patients have the right to expect privacy and confidentiality of their protected health information. 4.1.7.2 The right to expect that all communications and other records pertaining to be kept confidential. 4.1.8 Outcomes of Care, Including Unanticipated Outcomes Patient, and when appropriate, their families are informed about the results of treatment, including any significant unexpected patient care outcomes or when the outcomes differ significantly from the anticipated outcomes. 4.1.9 Pain Management 4.1.9.1 Pain assessment and management is considered an important part of a patient’s experience and care plan. 4.1.9.2 Procedures and resources are in place to assure that pain for all patients is recognized and addressed appropriately. 4.1.10 Patient complaint 4.1.10.1 The Center has a system to receive and address all patient complaints, whether the complaints are written or verbal, or are received by telephone or in person. 4.1.10.2 Individuals who file complaints are notified, once the investigation has been completed, of the outcome of the inquiry. 4.1.11 Security Reasonable security is maintained for patient through the center security service. 4.2 PATIENT / Family RESPONSIBILITIES: 4.2.1 Follow all Instructions mentioned on Patient’s Guidelines Booklet. 4.2.2 You have the responsibility to provide accurate and complete information concerning your present complaints, past illnesses and hospitalizations, and other matters relating to your health. 4.2.3 To follow the treatment plan established by your physician & Medical Team , 4.2.4 Is responsible for your actions if you refuse treatment or not follow the physicians’ orders. 4.2.5 Notify your physician, the Head Nurse, or the Social Worker representative of any dissatisfaction you may have in regard to your care at the center. 4.2.6 Be considerate of the rights of other patients and center personnel, and assist in the control of noise, smoking, and other possible sources of unnecessary disturbance and/or discomfort. 4.2.7 Show respect and consideration for other patients, visitors, and center Properties. 4.2.8 To sign the informed consent after you received enough information for procedures that may be needed or in case you insisted to be discharged against medical advice, and other forms requested by the center. 4.2.9 Be aware that center is committed to high standards of care and hospitality for patients and their families. 5. RESPONSIBILITIES 5.1 The Center Director: Will ensure compliance in Patient Rights & Responsibilities by all staff. 5.2 Head of Departments are responsible to: Ensure staff awareness and compliance to this policy. 5.3 Patient Experience coordinator: Raising and monitor implementation of this policy through daily round and patient feedback. System and complaint system. 5.4 All Center staff: It is the responsibility of every member of the Healthcare Team to identify and adhere to patients’ rights and responsibilities.
