Tanutan RN, LPT, Maed, PhD. and I will be your instructor for NRG 203: CARE OF MOTHER, CHILD, AND ADOLESCENT (WELL CLIENTS), SCHOOL AGE CHILD. Before we begin, allow me to introduce myself. I am Sir Jong, a Part-time faculty member of the Nursing Program of Davao Doctors College, Inc., I am a graduate of Bachelor of Science in Nursing at Davao Doctors College, I am also a College Professor for 3 years at Davao Central College handing subjects such as Biology, General, Organic and Nonorganic Chemistry, and Analytical Chemistry, Physics and Literature subject. I had been a Nursing Review Lecturer at Our Lady of Perpetual Help Review Center and GLOCAL Review Center handling Obstetric Nursing, Pediatric Nursing, and Communicable Diseases concepts. College of Allied Health Sciences Nursing Program Introduction to the Course The course NRG 203: Care of Mother, Child, and Adolescent: Growth & Development of a School Age Child deals with concepts, principles, theories, and techniques of nursing care management of well children at the Toddler stage. In this course, you are expected to provide nursing care to children utilizing the nursing process. Presented below are the specific details of this course: Course Number: NRG 203 Course Credits 9 units (4 units Lecture, 5 units RLE) No of Hours (Lab): Placement: Second Year, First Semester Prerequisite: NRG 105, NRG 106 Schedule: THU, FRI, SAT 7-3 GOOGLE CLASSROOM: Classroom: CLASS CODE: Requirements in Completing the RLE Activities Since most of our sessions for this semester will be delivered through distance learning activities, the submissions will also be primarily done online, albeit considerations will be extended to those with extreme limitedness in connectivity. To do this, you need to have access to the following applications: 1. Microsoft Office/Google Docs 2. Microsoft PowerPoint/Google Slides 3. Google Classroom or Quipper (Will depend on the instructor in-charge) 4. Google Meet (or any other teleconferencing app, duly agreed by the whole group) 5. Internet connectivity Please expect that you might need to print some parts of the worksheets in this module (i.e., documentation and transcribing parts). If any difficulties in participating in synchronous classes are encountered, inform your instructor right away! For absences, secure an excuse slip from your RLE supervisor. Preparation of PPT Case Presentation Weekly Quiz and Discussion of Presentation Module and Quiz Answers Orientation of Next Week’s Activity Now that you are done acquainting yourself with the instructor and the course itself, please proceed to Module 9: Care of the Mother, Child and Adolescent (Well-Clients): Gender and Development of a School Age Child Term: Second Semester S.Y. 2024-2025 Dates: Class Schedule: MTW/THFS: 7am-1pm/1pm-7pm No. of Hours: 6 HRS/DAY Instructions The case analysis manuscript may be handwritten or computer written. Observe pagination. Once done, send the compilation of your group’s answers to your clinical instructor. After the submission, prepare a PPT presentation for your case presentation slated for Days 2 and 3 of your RLE classes. Each member of the group is expected to present during the case presentations. Learning Outcomes At the end of this module, you are expected to: 1. Describe the normal growth and development of a School age child as well as common parental concerns. 2. Assess a school age child for normal growth and development milestones. 3. Formulate nursing diagnoses related to school age child growth and development or parental concerns regarding growth and development. 4. Identify expected outcomes for nursing care of a school age child. 6. Plan nursing care to meet a school age child’s growth and development needs, such as anticipatory guidance to prevent problems such as sleep disturbances. 7. Implement nursing care to promote normal growth and development of a school age child, such as discussing infant developmental milestones with parents. 8. Identify areas related to the care of an infant that could benefit from additional nursing research or application of evidence-based practice. 9. Integrate knowledge of school age child growth and development with the nursing process to achieve quality maternal and child health nursing care As you start with this module, you are free to consult and coordinate with your assigned clinical 😊 instructor. College of Allied Health Sciences Nursing Program Case Scenario: School Age Child with Urinary Tract Infection Course in the Ward: Chimmie, a 12-year-old child was brought to the hospital, parents explaining that their child has experienced five attacks of acute cystitis, all characterized by dysuria, increased frequency, and urgency. Parents reported that the child has “dirty” practices in performing personal hygiene whether at home or outside. Each case was diagnosed on the basis of the clinical picture and a laboratory urinalysis finding of bacteriuria. Lab tests history indicated that the first, second, and fifth infections were caused by Escherichia coli, while the third infection was caused by an enterococcus and the fourth infection was caused by Proteus mirabilis. Each infection responded to short-term treatment with trimethoprim-sulfamethoxazole. The recurrences occurred at intervals of 3 weeks to 3 months following completion of antibiotic therapy. For the past two days, she has once again been experiencing dysuria, increased frequency, and urgency, so the child’s parents decided that she goes to see her physician. Her vital signs are T = 37.2°C P = 100/min RR = 18/min BP = 90/60 mm Hg. Physical examination reveals a mild tenderness to palpation in the suprapubic area, but no other abnormalities. The most appropriate tests would include urinalysis with microscopic evaluation of clean-catch urine for bacteria and pyuria, a urine culture, and a CBC with differential. Laboratory tests indicate a Hgb of 13.6 g/dL, Hct 40.7%, MCV 84, and WBC count 10,910/microliter. White blood cells and bacteria are evident in the urine sediment. A urine culture indicates approximately 106 bacterial cells/ml. A Gram stain of the urine reveals Gram-positive cocci. The Gram-positive bacterium is isolated and is found to be catalase positive and coagulase negative. At present, the coagulase-negative Staphylococcus species that is mostly likely to cause cystitis in a young woman is Staphylococcus saprophyticus. Patient was then advised for admission. Hooked to D5LR 1 L @ 100 ml/hr. Laboratory tests was also ordered. Vital signs were taken and recorded as follows: BP: 85/55 mm Hg PR: 110 bpm RR: 23 cpm T: 37.9ºC UA result revealed positive trace of leukocytes thus advised to increase oral fluid intake and were given medications. College of Allied Health Sciences Nursing Program What to Do? To immerse yourself in the care management of your patient, let us do some detailed description of your patient care tasks. Using the Case Scenario assigned to you, you are expected to perform the following: 1. Fill-up the needed data based on the given scenario/case. 2. Conduct a history taking and physical assessment of your patient. Discuss the health history of the patient narratively, in chronological order as much as possible. Use the DDC Health Assessment form for documenting PA findings. 3. Discuss the developmental milestones of your patient in chronological order in terms of: a. What can the child do at this ages? b. What does the child understand? c. How will the child interact with others? 4. Interpret the therapeutics done for the patient: doctor’s orders, laboratory and diagnostic tests done, and surgical procedures to be performed. Use format presented. 5. Present your drug study and IVF data. One drug study per student. Transcribe the IVF and medications in the appropriate sheets. 6. Formulate a patient-centered care plan for your patient. Integrate in your care intervention significant bioethical and legal standards of patient care. One NCP per student. 7. Document the care given by making a nurse documentation following the FDAR format (individual). 8. Clinical Reasoning Questions - Collaboration: You are a nurse at a pedia ward and you are assigned to take care of your 9-year old client named Yonna. Yonna was diagnosed with Urinary Tract Infection and was instructed to avoid foods with artificial sweeteners, juices, and junk foods. Meanwhile, while you were doing your rounds in the AM shift, you noticed a big paper bag in Yonna’s bedside. It was full of empty packs of junk foods and juices. When Yonna’s older sister noticed you looking at the bag, she immediately got it and threw it away. What is your response in a situation like this?
