Obtunded - less than full alertness (altered level of consciousness), typically as a result of a medical condition or trauma. 3. Battle's sign - bruising behind the ears, indicative of a basilar skull fracture 4. Step off deformity - obvious abnormality in alignment between two vertebrae 5. Brudzinski's sign - sign of meningitis; positive when a patient's legs lift involuntarily when lifting a patient's head 6. Carotid bruit - turbulent blood flow heard with auscultation over the carotid artery. (Due to carotid narrowing or plaque) 7. JVD (jugular vein distention) - this is bulging of the external jugular vein which indicates increased blood volume and usually congestive heart failure (CHF) 8. lymphadenopathy - enlarged lymph nodes 9. Meningismus /Meningeal Signs - exam findings concerning for Meningitis 10. Tracheal deviation - a shifting of the trachea to either side of the midline of the neck caused by the buildup of pressure inside the chest 11. Tenderness - increased pain to palpation 12. TM (tympanic membrane) - eardrum. 13. Mastoid Tenderness - tenderness over mastoid bone (located posterior to ear) 14. Conjunctival injection - redness of the conjunctiva 17. Conjunctival exudate - pus-like material in the eyes, usually indicates conjunctivitis 18. Conjunctival Pallor - inside of the lower eyelid appears pale 19. Corneal abrasion - an injury, such as a scratch or irritation, to the outer layers of the cornea 20. EOM - extraocular movement observed as the eye moves through the different directions. Typically checked when a physician has a patient track their finger without moving their head. 21. Hyphema - blood in anterior chamber of the eye, as seen during the fundoscopic exam 22. Nystagmus - involuntary rapid eye movements. 23. Papilledema - swelling and inflammation of the optic nerve at the point of entrance into the eye through the optic disk 24. Periorbital - pertaining to the area surrounding the socket of the eye 25. PERRL - pupils equal, round, reactive to light. checked when a physician briefly shines a light in the patient’s eye, to look for pupil dilation. 26. Scleral icterus - yellowing of the sclera due to jaundice 27. Subconjunctival hemorrhage - bright red patches in conjunctiva of eyes due to ruptured blood vessels 28. Nasal flaring - enlargement of nostrils with breathing (sign of respiratory distress) 31. Mucus membrane (mucosa) - the membranes of the mouth; inner cheeks, tongue, gums, etc. 32. Malocclusion - any deviation from the normal positioning of the upper teeth against the lower teeth 33. Pharyngeal erythema - redness in the back of the throat 34. TMJ - temporal mandibular joint. 35. Tonsillar exudate - pus on the tonsils, indicating infection of the tonsils 36. Trismus - inability to open the jaw due to pain 37. Cervical motion tenderness - CMT; elicited by moving the cervix often indicative of PID 38. Cervical os - opening of the cervix to the uterus 39. Lesion - a pathologic change of the tissues due to disease or injury 40. Rales – Crackles: bubbling or noisy sounds caused by fluids or mucus in the air passages 41. Retractions - visible sinking of the area between the ribs and in the neck during inhalation, a sign of respiratory distress 42. Tachypnea - rapid breathing, a sign of respiratory distress. 44. Wheezes - continuous high-pitched whistling sounds produced during breathing heard on auscultation 45. Rhonchi - coarse, low-pitched breath sounds heard in patients with chronic mucus in the upper airways. 46. Friction rub - described as grating, scratching sound of the heart indicative of pericarditis 52. Gallop - abnormal heart sound that mimics the gait of a horse; related to abnormal ventricular contraction 53. Drawer Sign - a diagnostic sign of a torn or ruptured ligament. The positive anterior drawer sign is the test for the anterior cruciate ligament (ACLS). The posterior drawer sign is the test for the posterior cruciate ligament (PCL) 54. Edema - swelling 55. Dysconjugate gaze - failure of the eyes to turn together in the same direction 72. Dysphagia - difficulty swallowing 73. GCS - used to assess level of consciousness 74. Oriented - being aware of who they are, where they are, and the current time; a patient who is aware of all three is "oriented x 3" 75. Pronator drift - involuntary turning or lowering of forearm when outstretched 76. Somnolent - sleepy, drowsy 77. Excoriation - abrasion produced by scratching or scraping 78. Blanching - when whitish coloration of the skin remains longer than normal after pressure is applied on an area of the skin 79. Cyanosis - a bluish discoloration of the skin resulting from poor circulation or inadequate oxygenation of the blood. 80. Diaphoresis – sweating, even at rest 81. Fluctuance and Induration - boggy, soft area; sign of abscess. 82. Purpura - the appearance of multiple purple discolorations on the skin that are palpable caused by bleeding underneath the skin 88. Capillary refill - the amount of time it takes for capillaries that have been compressed to refill with blood 89. NIH scale - measures 11 different neurological factors 90. Heme positive (Guaiac positive) - blood in stool 91. Ligamentous laxity – degree of looseness in the ligaments of a joint. 92. Orthopnea- shortness of breath when lying flat. 93. Pallor- pale appearing skin. 94. Asterixis- tremor of the wrist when the wrist is extended. 95. Apneic- pertaining to the absence of breathing.
