Russian Current 1 Russian Current Russian current is an even polyphasic alternating sinusoidal medium frequency(2500HZ), applied as series of bursts(short pulse), which is developed by Russian physiologist named Yakov Kots 1977. Russian current is most often used to strengthen muscles. it is also commonly used to re-educate muscle groups after a nerve pathway has been damaged, and in the case of someone who has had a stroke It provoked much interest because the very successful Russian Olympic teams were using it in addition to their usual training methods and it was suggested that its use lead to significant (30-40%) gains in muscle strength. Characteristics On/Off Ratio: The on-off ratio is defined as the ratio of the time during which there is stimulation (contraction) to the time during which there is no stimulation (no contraction). Set as 10ms:10ms. Duty cycle: The duty cycle is the proportion of on-time to the summation of both the on and off time expressed as a percentage. For example 10:10 sets up a duty cycle of =10/(10+10) ×100= 50%. Russian current produced with a duty cycle of 50%. Ramp-up and down: Setting the ramp time is very important to evoked contraction similar to muscle contraction. As the ramp allows a gradual increase of intensity (surge) with associated gradual increase of muscle contraction. Set for 1-2second. Duration of treatment is recommended to be : 10 minutes Characteristics of Russian Current Carrier Frequency: 2500 Hz ( 2.5KHz). Waveform: Polyphasic sinusoidal waveform. Current Amplitude: The maximum current amplitude is 100mA Clinical used at 70mA. Bursts Frequency: 50 hz Burst Duration:10ms On/Off Ratio : 10ms:10ms Duty cycle : 50% (duty cycle =10ms on time /(10 on time+10 of time) ×100= 50%. NB: sometimes these parameters cause muscle fatigue because it is very strong. Russian Current Characteristics Training protocol with Russian current to prevent muscle fatique: (10/50/10) 10 = one muscle contraction lasting for 10 seconds 50= off time (no contraction) lasting for 50 seconds. 10=cycle repeated for 10 times. Amplitude/ Intensity: (70mA) strong but comfortable muscle contraction Pulse rate (frequency): 50 – 70 Hz (tetanic muscle contraction) Duration of treatment : 10 minutes. Physiological Effects of Russian Current Physiologic Effects Depends on: 1-The total number of bursts delivered per second. 2-Russian current has a capacity to depolarized : Sensory nerve fibers Moderate prickling sensation, Motor nerve fibers Muscle contraction This contraction is Painless due to sensory nerve blocked of pain gate Therapeutic Clinical Uses Improve muscle strength Amplitude: Tetanic muscle contraction. Pulse rate (Burst freq): 50 Hz Rule applied : (10/50/10) Treatment time: 10 mins/session/3-5days for 6 weeks Therapeutic Clinical Uses Reduce edema : Amplitude: small visible muscle contraction. Pulse rate (Burst freq): 35-50 Hz Rule applied : On : Off Time will be (on time of 1-2 secs and off time 1-2 secs) to promote muscle pumping Treatment time: 10-15 mins/session ,3-5 days/wk for 6wks. indications The only indication for use of Russian current is to: Strength the muscular system of healthy and athletic persons : Post-knee ligaments surgery Post-arthroscopic knee surgery. Following ligament sprain Post casting Contraindications Over the anterior cervical area Over the lumbar or abdominal area during pregnant Over hemorrhagic area Over neoplastic area Over metallic and electronic implant Over area of impaired skin sensation Thank You 1 9
