Medications that look or sound similar. Can lead to confusion and mistakes. LOOK ALIKE SOUND ALIKE Why They Pose a Risk Misidentification: The similarity in appearance or sound can lead to confusion, especially under stressful conditions or when the medication is not clearly labeled. Incorrect Administration: If a medication is mistaken for another, it could result in the wrong dosage or administration route Impact on Patient Safety The wrong medicine could make patient feel worse or even cause allergies. You might lose trust: If they have a bad experience, they will not trust doctors or hospitals anymore. It will cost them more: Mistakes with medicine can make patients pay more for healthcare. What to do? Store separately Bold labeling Avoid abbreviations of drug names Write legibly - Include the indication for the medication. Use both generic and brand names is preferred. Organize equipment based on function or frequency of use. Place frequently used items within easy reach. Secure equipment to prevent accidental falls. Regular Checks Use checklist Check for expiry Regular inspection Ongoing training on crash cart usage and maintenance.
