The acid comes from the bacteria from the plaque bio lm & the fermented carbs acting over tooth surface. Formation: saliva coats on the tooth surfaces. Glycoprotein that’s present in the saliva will coat the surface (which is a sticky surface). That makes the carbs/bacteria to stick over it. Even if you brush your teeth, there’ll be bacteria (oral inhabitants) stable inside our oral cavity. So even if you brush you teeth, when the saliva coats your teeth, the bacteria will adhere and coat the tooth surface due to the presence of the sticky surface. So if you don’t remove the plaque for a long time due to habits like eating a lot of sugar or not brushing teeth properly, then that will settle over there, and if left for a long period of time, then you’ll get these reactions (like fermentation of carbs) which will lead to acids which will cause demineralization and proteolytic destruction. When it’s restricted only to acid demineralization, that means this is gonna be only on the enamel which is possibly a reversible process. But when proteolytic destruction happens, this is something that can happen when it’s irreversible. When demineralization happens, that means you can always reminerlize the area by applying reminerlization solution. If you nd the area demineralized, you will be able to identify that and apply a reminerlization solution or tooth paste, the tooth can get reminderlized. You can deposit the minerals over there. But if there’s proteolytic destruction, then it’s irreversible. At that stage, the Caries will be reached at the irreversible stage. If the patient can identify the caries then it’s a progressive caries. Early caries can only be identi ed by a professional.
