Laboratory Test Preparation In HEALTHONLINE, we can also check the laboratory test details, Cut off, Running Time and Releasing of the test Results. NOTE: The price of test is not included or indicated in the website. Branches Under Branches, you will be able to check the details of the branch including the address, contact numbers, office hours, contact person , available services, and affiliated HMOs. Services and Packages You can check services and packages in the website. HMOs You can check what HMOs are acceptable in Hi – Precision branches in our website. Patient need to present HMO card and valid ID for consultation, additional doctor’s request with diagnosis from accredited doctor for laboratory & imaging request. Patient can send request for HMO approval using HMO Approval in our website or can also proceed to our preferred branch for an approval. Online HMO Approval Patients are required to upload valid ID, HMO card, and test request issued by an HMO accredited doctor with corresponding diagnosis. Unmatched details may result in delay of approval. HMO Coordinators will contact patient to verify some information if necessary. Forms should be submitted at least 2 days before the scheduled date and that approved requests are only valid within 3 days from approval date. Patients will receive a confirmation email once request has been successfully received. Note: Online service is UNABLE to process approvals for STAT or Urgent requests as well as procedures / services that require confirmation of availability / appointment (Ultrasound, 2D Echo, Treadmill/Stress Test, Stress Echo, Vascular, Holter Monitoring, Ambulatory Blood Pressure Monitoring, Doctor’s Consultation, and Pap Smear Collection). Home Service Patients may call 8741-7777 , press 1 for inquiry, press 2 for home service appointment Patients may also download Hi-Precision Diagnostics App and set an appointment Forms Patients may download the forms online for : Patient Data Sheet Special Test Form (RAPTH9-RAPAMUNE) Case Investigation Form (CIF) A reference number will be generated once patient accomplished the form. Take a screenshot of it, present it to our staff on the day of visit. Vaccine Store Patients may order vaccines online instead of going to HPD branch for the payment. Advise patient to consult a doctor prior to availing of the vaccination service. If patient do not have her/his own doctor, HPD branches have available doctors for consultation. Inform patient to call chosen branch for appointment. Vaccines are to be administered by one of Hi-Precision Diagnostics' Primary Care Physician (PCP). Availing of vaccination service is deemed final and is not eligible for refund or exchange unless the vaccine is defective or in poor condition. Vaccines are by order basis. TAT is 5-7 working days upon ordering. Customer Care Specialist of branch will call patient once vaccine is delivered to the branch. Refund is only allowed if vaccine ordered is Out of Stock and client is not willing to wait. Only the VP- Purchasing can approve and process the refund thru Paypal. Inform patient that he/she will receive a notification once payment is refunded. Endorsement system Endorsement system is for our corporate accounts. HR of the company may send endorsement to HPD branches using this service. Authorized HR of the company is provided with username via email for login. HR need to fill out the required details in order to proceed with the request for pre-employment or annual physical exam. Employees are required to present valid ID on the day of visit to HPD branch. Online Result From our website https://www.hi- precision.com.ph, the patient can also view their laboratory test result. By using their PID# and surname as password to log in. Patient can find the PID# from the upper left part of the Order form. PID# Ex: D0524804. In case patient forgot their PID or lost their order form and call for assistance, CSR need to verify the details of patient first before giving the PID over the phone. Need to get the full name of patient and the date of birth. In case caller is not the patient, need to verify the relationship of the caller with the patient. Corporate accounts can also view results online using the login credentials provided by our IT Team to Online Result Note: Results of BracAnalysis, Endopredict, Food Intolerance, Hair Drug Test Panel, Horizon, LiverFast, MyRisk, Omega 3, Oncovantage, Panorama, Pre-Natal Gender Test, & Prolaris are not available online. Patient can pick up the hard copy of result in tHPD branch. Online Result Patient can download and print laboratory test result. From main home, select patient history, enter patient ‘s PID then search. Under information, open the icon to check Test the status of the results. Status: Complete – it means the laboratory test is available or ready for pick up. No Specimen Received, Running or Scheduled Running - it means the laboratory test still on process or ongoing or with running day. Note: For No Specimen Received, Running or Scheduled Running are not allowed to relay this to patients. Customer Service Representative should inform the patient that test is still on process, or may check the Laboratory test preparation in HPD website for the running day of the test with running or Laboratory Result The test goes through different statuses before it becomes complete: NO SPECIMEN RECEIVED - From the time that it is encoded until the Laboratory receives the specimen RUNNING OR SCHEDULED RUNNING - Once received, the test remains in this status until the Laboratory Officer authorizes the result. COMPLETE - Once authorized the status displays the result of the test Laboratory Result Patient’s cumulative results can include the past five results of the same test (Applicable to tests with numeric results only). Laboratory Result Results are displayed by asterisks *** for calculated results with negative (-) or invalid values. When the mouse is hovered over the icon beside the ***, a pop-up description box appears showing the result. However, they may be also used as an identifier that these tests have been rejected or that these tests are complete. Patients are not allowed to get copy of FME directly from Hi-Precision. Patient may coordinate with the Agent of Insurance to request for the copy from underwriting department. Tests performed by partner institutions depend on the agreement with the Releasing Specialist as to where results (i.e. audiometry) shall be claimed. Imaging Procedure and Preparations CT Scan with Contrast It uses a combination of X-rays and a computer to create pictures of organs, bones and other tissues. To help them appear clearly, a special dye called a contrast material is needed. Patient needs to fast for 4 – 6 hours, depending on the procedure. No metformin should be taken 48 hours before the procedure. Imaging Procedure and Preparations Mammography Do not wear deodorant, talcum powder or lotion under the arms or on the breasts on the day of the exam. The best time to have a mammogram is 1 week after the last day of menstrual period when the breasts tend to be less tender, thus the patient will feel less pain. Inform the X-Ray Technologist if the patient has breast implant/s in order to avoid rupturing the implant/s. Bring previous mammogram film for comparison if any. Imaging Procedure and Preparations Treadmill Stress Test This test is used to provide information about how the heart responds to stress. Normal rest the night before the test. Bring usual medications and comfortable clothes. Avoid coffee, tea, chocolate and smoking at least 6 hours before the procedure. Light meal or snack can be taken 2 hours prior the procedure.
