STANDARD OPERATING GUIDELINE NOTICES TO PROPERTY OWNERS AND OCCUPANTS Date Issued: March 29, 2019 Rescinds: December 23, 2013 Section: Incident and Emergency Operations File Code: G-NOTI PURPOSE To provide all Toronto Fire Services personnel with a guideline for completing various forms that may be left on scene at an emergency incident indicating actions taken by TFS personnel. RESPONSIBILITY All personnel are responsible to be familiar with and to follow the Toronto Fire Services guideline regarding the distribution and completion of hand written forms that may be used at an emergency incident. The apparatus driver is to ensure copies of the Immediate Action form and Action Taken form are carried on the apparatus as part of the regular inventory. When any forms are issued at an incident this information is to be included in the Emergency Incident Report. Where appropriate check the Fire Prevention Notes box and include a narrative as to why the form was issued. GUIDELINES 1. Immediate Action Form – Triplicate 1.1. The Immediate Action form booklet can be requisitioned from the Quartermaster Section. 1.2. The three copies of this form are to be distributed in the following order: a) Original to the property owner/building representative i. Provided directly or, ii. Left in a visible location (Fire Alarm Panel, CACF, inside front entrance, etc) b) Canary copy to the Fire Prevention Division office in the Command where the call occurred c) Pink copy to the District Chief in the District where the call occurred 1.3. The Officer completing an Immediate Action form should indicate in the Emergency Incident Report that a form was issued and to whom or where it was posted. 1.4. The Immediate Action form is to be used: a) If the building requires immediate action by the Fire Prevention Division for example: File Code: G-NOTI Page 1 of 2 STANDARD OPERATING GUIDELINE i. No Fire Safety Plan ii. Hoarding iii. Fire Alarm or Sprinkler System shut down iv. Blocked Fire Department Access (e.g. construction equipment) v. Blocked Exits (and where doors are chained, padlocked, etc) b) For all Level II or Level III Elevator Entrapments where TFS have de- energized (performed mainline disconnect) and lockout/tagout. 2. Action Taken Form 2.1. Copies of the Action Taken form can be printed from the TFS Intranet _ Forms Section. 2.2. The completed copy of this form is to be provided to the property owner/building representative either: a) Provided directly or, b) Left in a visible location (Fire Alarm Panel, CACF, inside front entrance, etc.) 2.3. The Action Taken form is to be used when: a) TFS has shut down any building services requiring qualified 'third party' reconnection (electrical, gas, solar, etc.) b) TFS has caused damage during mitigation of an emergency incident; i. To a building. ii. To other property such as a vehicle, container, etc. 2.4. The Officer completing an Action Taken form should indicate in the Emergency Incident Report that a form was issued and to whom or where it was posted.
